Job Description Summary:

The Decedent Affairs Navigator serves to ensure accurate processes are followed for management of deceased patients (including pregnancy losses) between death and final disposition. This includes coordination with internal partners in Nursing, Pastoral Care, Patient Transport, Protective Services, and Pathology. External processes are coordinated with organ procurement organizations, county medical examiners, law enforcement agencies, extended care facilities, funeral homes, families, and other third-party organizations. The Decedent Affairs Navigator will proactively coordinate and communicate to ensure timely and appropriate disposition and monitor system-wide morgue management under the leadership of the Advisor/Manager of Decedent Affairs. This position is currently a work from home function and requires the associate to provide a confidential work environment and high-speed internet service. Work schedule is variable including night, weekend, and holiday coverages.

Responsibilities And Duties:

55% Customer Service:
Proactively coordinating the processes required for timely and accurate disposition of deceased patients and pregnancy losses with Nursing, Pastoral Care, Patient Transport, Protective Services, Pathology, and other internal departments.
External processes are coordinated with organ procurement organizations, county medical examiners, law enforcement agencies, extended care facilities, funeral homes, families, and other third-party organizations.
The Decedent Affairs Navigator also monitors system-wide morgue management and assists funeral homes with the necessary information for death certificate completion including signing physician information.
Various technology applications will be utilized for communication not limited to Microsoft Office Teams, Cisco phones, Secure chat in Epic, and occasional use of a personal cell phone with internal partners.
Cultural/religious understanding and sensitivity are required.

30% Documentation:
Ensuring accurate documentation in Decedent Affairs database and Epic/CareConnect for all steps in the decedent care process.

Assisting with legal next of kin searches under the direction of the Decedent Affairs Advisor/Manager.
5%
Communicating difficult cases, including next of kin or indigent matters, to the Decedent Affairs Advisor/Manager.

Other duties as assigned.
