Overview

The RN is responsible for providing clinical care, patient education, and care coordination for pediatric and adult patients with bleeding and clotting disorders. This role requires strong communication skills, the ability to collaborate within a multidisciplinary team, and a commitment to supporting patients across the lifespan. 


The Nurse Coordinator is an integral member of the multidisciplinary team, providing developmentally appropriate care to patients and families of all ages, with a primary focus on adult care and an emphasis on bleeding disorders, as well as clotting and vascular disorders. The Nurse Coordinator serves as a liaison among patients and families, providers, clinical staff, pharmacy, and scheduling/registration personnel to ensure safe, efficient patient flow.


The primary role of the Nurse Coordinator is to ensure seamless coordination of patient care related to bleeding and clotting concerns to support the best possible outcomes. 


Travel to other sites may be required as needed.


Schedule:  Full-time, 37.5 hrs., 8:30 AM – 5PM. Occasional weekends and Recess period may be required.


Required Skills and Abilities

1. Excellent interpersonal communication skills, demonstrated telephone and organizational skills. 

2. Proven ability to set priorities, detail-orientated, and ability to multi-task and work in a multidisciplinary team environment. 

3. Proven ability to resolve problems in an efficient and effective manner.

4. Computer proficiency with MS Office Suite, especially Word, Excel, etc.

5. Flexible hours to accommodate clinics and section needs. Travel within CT may be necessary 


Preferred Skills and Abilities

Infusion and phlebotomy skills preferred. 

Principal Responsibilities

 

1. Plans, directs, and implements the daily activities of patient care. 2. Contributes to developing, implementing and evaluating policies and procedures related to nursing. 3. Contributes to developing, implementing , evaluating and revising research protocols related to nursing as indicated. 4. Provides patient and family education on appropriate disease process to include, but not limited to signs and symptoms, treatment, purpose and side effects of medications. 5. Follows-up on patient lab results and work with clinical faculty to develop a follow-up plan that is communicated to the patient and family. 6. Serves as the primary contact for all patients and their family concerning management and treatment of their disease by arranging for prescription refills, telephone follow-up, clinical visits, home care visits, and emergency room visits. 7. Collaborates with other health care professionals and service providers to ensure optimal patient care. 8. Creates and maintains effective, positive customer service relationships by identifying patient and family needs, ensuring that needs are met and responding appropriately to difficult interactions. 9. May perform other duties as assigned. Required Education and Experience Bachelor's Degree in Nursing and 3 years of experience or an equivalent combination of education and experience. Required License(s) or Certification(s) Licensure as a Registered Nurse in the State of CT.
