Core Responsibilities:

Patient Assessment

  • Collaborate with physicians to establish and evaluate treatment programs, care plans, goals, and discharge plans tailored to patient needs.
  • Develop individualized plans of care in conjunction with patients and their families.
  • Create realistic discharge plans that incorporate patient and family goals.

Treatment Planning and Provision of Care

  • Implement physical therapy treatment plans effectively.
  • Select appropriate activities to help patients progress toward their goals.
  • Revise treatment goals and plans based on ongoing patient and family input.
  • Provide continuous education and training for patients and families.
  • Offer discharge instructions, follow-up, and referrals to community resources as needed.
  • Serve as a resource for staff, families, and physicians regarding rehabilitation services and equipment.

Documentation

  • Document evaluations and care plans according to departmental guidelines.
  • Record each treatment session in line with established protocols.
  • Complete all required documentation, including family education and discharge summaries.

Required Qualifications:

Education:

  • Bachelor’s, Master’s, or Doctorate in Physical Therapy required.

Licenses and Certifications:

  • Physical Therapist license required.
  • Basic Life Support (BLS) certification required.

Experience:

  • Relevant experience in physical therapy required.

Skills:

  • Strong communication skills.
  • Customer service orientation.
  • Team collaboration abilities.
  • Basic computer proficiency.
