Job Summary:

The Cystic Fibrosis Coordinator is responsible for the comprehensive coordination of care for patients with cystic fibrosis (CF) at the University of Mississippi Medical Center. This role involves working collaboratively with a multidisciplinary team to ensure the delivery of high-quality, evidence-based care and support for CF patients and their families. The Coordinator will also be involved in patient education, clinical research, and quality improvement initiatives.

Education & Experience

Must be licensed as a RN for two (2) years. Experience in cystic fibrosis or pulmonary care is preferred.

Certifications, Licenses or Registration Required:  Valid RN license.

Knowledge, Skills & Abilities

Work closely with pulmonologists, nurses, dietitians, social workers, respiratory therapists, and other healthcare professionals.

Participate in regular multidisciplinary team meetings to discuss patient cases and care strategies.

Ensure that all team members are informed of patient status and any changes in care plans.

Provide education to patients and families about cystic fibrosis, treatment options, and self-management strategies.

Organize and conduct patient and family education sessions and support groups.

Develop and distribute educational materials tailored to the needs of CF patients and their families.

Assist in the recruitment and enrollment of patients in clinical trials and research studies.

Collect and manage data for clinical research projects.

Participate in quality improvement initiatives aimed at enhancing the care and outcomes of CF patients.

Stay updated on the latest research and advancements in cystic fibrosis care.

Responsibilities

  • Serve as the primary point of contact for CF patients and their families.
  • Coordinate outpatient and inpatient care, including scheduling appointments, diagnostic tests, and follow-up visits.
  • Facilitate communication between patients, families, and the healthcare team to ensure continuity of care.
  • Assist in the development and implementation of individualized care plans.
  • Monitor patient progress and make necessary adjustments to care plans in collaboration with the healthcare team.
  • Maintain CF Foundation patient registry.
  • Maintain accurate and up-to-date patient records.
  • Ensure compliance with regulatory requirements and accreditation standards.
  • Prepare reports and presentations for departmental and institutional meetings.
  • Assist in the development of policies and procedures related to CF care.
