Summary:

The Data Integrity Specialist provides operational support to the Memorial Healthcare System’s Health Information Management (HIM) department. The Specialist is responsible for performing all duties necessary to ensure the integrity of health-related data documented in the electronic health record. This includes, but is not limited to, facilitating the correction of documentation that impacts patient care and addressing duplicate medical record numbers in accordance with Memorial Healthcare System’s policies and procedures. Chart correction responsibilities may occur outside the department’s normal business hours requiring on-call coverage.

Responsibilities:

Performs all other duties as requested.Corrects medical record documentation errors; performs Electronic Master Patient Index (EMPI) maintenance. Merges or unmerges medical records (patient duplicates and overlays); moves incorrectly assigned encounters to the appropriate medical record.Monitors patient identity electronic work queues and the EMR system to identify, resolve, and communicate documentation errors. Validates the medical record documentation error has been accurately resolved or remediated.Resolves Roster Management Interface Engine (RME) errors for patient populations loaded into the EMR for Population Health patient follow-up and monitoring. Includes reconciliation of patient identifiers (name, medical record number, date of birth, address, and payer member ID number).Creates batch releases for Auditors and outside requestors for EPIC CARE LINK (remote) access to medical records.Regularly communicates with the Director of Identity Management for collaboration, feedback, and ongoing training.Interacts with the electronic medical record (EMR) system, clinical providers, other end users, clinical departments, and Privacy to facilitate and follow-up on the correction of medical record documentation errors identified in the Memorial Healthcare System legal medical record.

Education and Certification Requirements:

Associates (Required)Registered Health Information Administrator (RHIA) - American Health Information Management Association (AHIMA), Registered Health Information Technician (RHIT) - State of Florida (FL)

Complexity of Work: This role requires critical thinking skills, effective communication, decisive judgment, and the ability to work with minimal supervision. Good organizational skills and strong follow-through capabilities. Must be able to meet deadlines, work independently, set priorities and maintain confidentiality. Must be able to work in a stressful environment and take appropriate action. Proficient in use of various software applications, electronic medical record systems, and Microsoft Office products (Word, Excel, PowerPoint). Knowledge of medical terminology, anatomy and physiology and regulatory guidelines. Required Work Experience: Two (2) years’ experience using an electronic medical record (EMR) system performing patient identity merges or chart corrections. In lieu of Associate’s degree, must have five (5) years of electronic medical record patient identity chart corrections experience.
