At Your Door: Visiting Healthcare Services is hiring a Lead Nurse Practitioner/Physician Assistant to work in Columbus, OH area! Our visiting provider group serves Assisted Living, Independent Living, and Skilled Nursing Facilities. PURPOSE          

According to prescribed policies and procedures of the organization including all applicable state, federal and accreditation regulations and under the general supervision of the Director Operations – Advanced Practice, the Lead Nurse Practitioner has the responsibility to effectively provide day-to-day leadership, support and guidance to the APP within their assignment, also known as their “pod”. The works with the Medical Director to ensure practitioners are providing quality of care to patients and access to care is available to all patients.

Qualified candidates for this position offer:

  • Education: Masters of Nursing from an accredited program and certification as a Nurse Practitioner or a masters in Physician Assistant/PA is required.
  • Experience: Minimum 3 to 5 years’ work experience as an Advanced Practice Nurse in a healthcare setting.  Previous supervisory experience preferred.
  • Mental: Must have excellent communication, comprehension, and interpersonal skills.  Must have the ability to speak, read, write and understand English, as well as the ability to make self-understood.
  • Skills: Must have good working computer skills with basic professional computer software (Microsoft Office suite, Windows, etc.) and the capacity for learning new software systems quickly.  Must be self-directed with excellent organizational skills and the ability to manage tasks and assignments concurrently.  Must have strong motivational skills and a high emotional intelligence quotient (EQ)
  • Licensure: Current Ohio RN License and Nurse Practitioner or Physicians Assistant in good standing.

ESSENTIAL FUNCTIONS

Management Responsibilities

  • Training and managing orientation schedule
  • Quarterly chart audits
  • Taking day to day phone calls re: clinical questions
  • Assisting with scheduling/assignment changes within their geographic area, including approving PTO (ensuring there is adequate coverage)
  • Will approve sending patients to hospital/ED vs treating in place
  • Assists with ensuring their team meets VBC KPIs
  • Assists in the evaluation and performance review of Nurse Practitioners.

Patient Panel responsibilities

  • Orders, performs, and interprets laboratory tests for screening and for diagnosing.
  • Prescribes and orders appropriate diet, medications and other forms of treatment such as physical therapy.
  • Performs therapeutic or corrective measures, including urgent care.
  • Refers individuals to appropriate specialist nurses or physicians or other health care providers.
  • Provides information, instruction, and counseling on health maintenance, health promotion, social problems, illness prevention, illness management and medication use.
  • Evaluates the effectiveness of instruction and counseling and provides additional instruction and counseling as necessary. Initiates and maintains follow-up of noncompliant patients.
  • Serves as preceptor for medical, nursing, NP, or physician assistant (PA) students.
  • Consults with other health care providers about established clients who have been admitted to hospital, home care, rehabilitation, or nursing homes.
  • Corresponds with insurers, employers, government agencies, and other health care providers about established clients as necessary.
  • Manages care of clients; develops plan of treatment and/or follow-up and monitors progress, determines when referral to another provider is necessary, makes necessary arrangements for further care, determines when hospital admission or emergency room visit is necessary, and determines when illness is resolved.
  • Assesses social/economic factors for each client, and tailors care to those factors.
  • Maintains familiarity with community resources and connects clients with appropriate resources.
  • Contracts with clients regarding provider responsibilities and client responsibilities.
  • Maintains an acceptable level of productivity to meet the facility goals.

NON-ESSENTIAL FUNCTIONS

  • Actively participate in meetings and committees as requested
  • Assist the VPs with new projects and assignments as necessary
  • Ability to be flexible with schedule and manage multiple assignments and projects simultaneously
  • Maintain excellent working relationships with community, regional and home office team members

EXPECTATIONS

  • Demonstrates and maintains a positive, professional manner, manages time efficiently and effectively, provides leadership as a role model to promote a team approach, collaborates and participates in policy creation and appropriate problem-solving methods.
  • Must adhere to all policies, procedures, terms and conditions set forth in the National Church Residences’ Employee Information Guide (EIG), the Code of Conduct, as well as any facility handbook, including but not limited to, corporate compliance, drug free workplace, safe work practices, all federal, state, local regulations and laws.
  • With respect to Resident Rights, ensures all care is provided with respect and dignity for the residents/clients, their families and the staff. Reports all complaints made by residents/clients and/or families to the appropriate supervisors, reports all allegations of abuse, misappropriation of funds/resident property and/or any other corporate compliance, drug free workplace, safe work practices, all federal, state and local regulations and laws.
  • Completes all annual education requirements timely, including but not limited to Compliance, Privacy, Security and HIPAA training and exhibits behavior as set forth by the Code of Conduct in the performance of their duties.
  • Performs other duties as assigned.
