Job Description

Corewell Health Urgent Care is looking for an experienced (1-3 years) Advanced Practice Provider (APP) to join their practice at the Rockford Integrated Care Campus caring for patients across the generational spectrum.  The urgent care group is 60+ APPs strong plus a handful of physicians. As a provider in the urgent care you will be credentialed at multiple locations and will typically work at 2-3. 

Family Nurse Practitioners or Physician Assistants required as we care for patients of all ages.

Team-based care is supplemented with medical assistants, radiology technicians, practice managers and access to consult all specialties.  Open 7 days a week, 8am – 8pm, located in Rockford, right off US 131.

Essential Functions

  • Functions in the role of physician assistant to coordinate comprehensive care delivery. Writes orders/prescriptions medications, durable medical equipment, physical therapy, laboratory, radiographic studies and consultations, coordinating diagnostic evaluations, follow-up exams, laboratory tests, referrals to home care, social services, and other community agencies, etc., in order to ensure appropriate continuity of care and coordination of services.
  • Obtains histories and performs physical exams.
  • Assesses the patients/significant other, health care team, school and other community educational needs and develops, implements, evaluates and revises teaching strategies/educational programs related to area of specialty.
  • Acts as a patient advocate, crossing multidisciplinary lines to meet patient needs. Communicates, collaborates, networks with and acts as a consultant to other members of the health care team within and outside Spectrum Health. Acts as a consult in area of expertise.
  • Maintains current knowledge and skills in areas pertinent to the delivery of urgent care medicine.
  • Able to perform all waived laboratory tests provided on premises in urgent care environment. Performs and assists with designated procedures.
  • Documents in the medical record and dictates information as appropriate in a timely manner. Completes all necessary documentation to provide accurate and efficient billing for services.

  • Provides input into the strategic planning, implementation and evaluation of service/program.

Qualifications

  • Required Master's Degree Graduate of an accredited Physician Assistant educational program
  • 3 years of relevant experience primary care experience preferred, but may be waived in cases in which sufficient relevant experience may compensate for lesser primary care experience Preferred
  • LIC-Physician Assistant - STATE_MI State of Michigan Upon Hire required
  • CRT-Physician Asst Certified (PA-C) - NCCPA National Commission on Certification of Physician Assistants Upon Hire required
  • CRT-Basic Life Support (BLS) - AHA American Heart Association 90 Days required Or
  • CRT-Basic Life Support (BLS) - ARC American Red Cross 90 Days required
  • CRT-Adv Cardiovascular Life Support (ACLS) - AHA American Heart Association If required by specialty or stress test performed. 120 Days required
