Overview

Provides operational support to the VNS Health affiliated medical practice, Medical Care at Home, to provide excellent care, customer service and meet contracted targets and goals. Works under general supervision. This is not a Medical Assistant position.

Compensation Range:$19.18 - $23.99 Hourly

What You Will Do

  • Provides assistance and information to clinical staff and management, patients, and their families.
  • Responds to inquiries and resolves routine to moderately complex issues; takes detailed documentation and escalates issues as needed to appropriate party, areas and/or program.
  • Communicates with Clinicians regarding availability and following program workflows, coordinates clinician schedules, priority need, and geographic area to maximize utilization effectiveness and efficiency.
  • Responsible for maintaining and updating all systems related to Clinician workflows, including Provider availability and capacity, referral outreach status, visit status, and geographic location maps.
  • Compiles and sends daily notifications to clinicians with patient information, including demographics, locations, and known clinical information including prior diagnoses (HCC) and Med Lists.
  • Conducts outreach calls to referred Health Plan members. Uses appropriate greeting, provides accurate information on reason for call and explanation of clinical services and instructions.
  • Registers patients in EMR. Obtains and verifies demographic, insurance and additional contact information.
  • Responsible for booking all patient related appointments, confirming appointments with patients, cancelling and rescheduling as necessary. Supports patients and clinicians in conducting virtual visits, proving portal instructions and links.
  • Assists with communication to the patient’s Care Manager and/or PCP, preparing and sending referral letters, clinician visit notes and follow-up as needed.
  • Assists management in insurance eligibility checks and initial and ongoing visit authorization prior to care being provided.
  • Documents all appropriate updates, phone encounters and actions taken in the electronic medical record for Medical Care at Home and ensures necessary documentation is in order with proper identification, signatures and dates.
  • Compiles information and prepares basic analyses of requested information or data.
  • Participates in special projects and performs other duties as assigned.

Qualifications

Education:

  • High School Diploma or the equivalent required
  • Bachelor's Degree preferred


Work Experience:

  • Minimum two years of general office, medical office or customer service-related experience required
  • Effective oral, written and verbal communication and customer service skills required
  • Intermediate computer skills, including Microsoft Office applications required
  • Bilingual skills may be required as determined by operational needs preferred
  • Mandarin or Cantonese speaking preferred
