Key Position Details:
This opportunity will be primarily remote with some in-person patient visits and IDT meetings.
Call Rotation: 1 in 5 nights + 1 in 4 weekends
Job Description:
Responsible for direct patient care by reviewing patient's medical history while collecting, prioritizing, and synthesizing comprehensive data pertinent to the patient's health or situation. Provides education, consultation or intervention to patients (treatment plan) and their families.
Responsible for the development, implementation and evaluation of programs and systems which facilitate care of the individual patient and family relative to symptom management, physical care, psychological support, and end-of-life planning. Provides evidence based care for patients with advanced progressive illness while promoting a healing environment.
Principle Responsibilities
- Direct Patient Care.
- Reviews patient's medical history while collecting, prioritizing, and synthesizing comprehensive data pertinent to the patient's health or situation.
- Evaluates, interprets, and coordinates information to make decisions and recommendations regarding patient care.
- Facilitates care of the individual patient and family relative to symptom management, physical care, psychological support, and end-of-life planning within an interdisciplinary care team.
- Provides evidence-based care for patients with advanced progressive illness while promoting a healing environment.
- Provides education, consultation or intervention to patients (treatment plan) and their families.
- Works collaboratively with patients to improve their health and well-being.
- Practices in a thoughtful manner including consideration for cultural diversity, individual autonomy and legal responsibilities.
- Charts in electronic format in accordance with licensure and scope.
- May provide care in an ambulatory clinic, inpatient or outpatient hospital setting.
- May participate in on-call rotation.
- Quality.
- Utilizes quality improvement tools to develop processes that promote continuous improvement in delivery of patient care, patient outcomes, or work flows.
- May participate in peer review and quality assurance.
- Other duties as assigned.
Required Qualifications
- Doctoral/Ph.D. level advanced degree in Doctor of Medicine, Doctor of Osteopathic Medicine or foreign equivalent from an approved school of medicine
- Completion of an Accreditation Council for Graduate Medical Education (ACGME) or American Osteopathic Association (AOA) accredited Residency training program in Family Medicine or Internal Medicine
- Completion of a 12 month ACGME or AOA training program in Hospice and Palliative Medicine, or meet the pre-requisite requirements to apply for the certification examination
- 0 to 2 years of demonstrated experience in designated specialty area
Licenses/Certifications
- Licensed Physician - MN Board of Medical Practice required if working in the state of Minnesota upon hire
- ACLS-BLS Tier 3 - Multisource required upon hire
- Licensed Physician - WI required if working in the state of Wisconsin upon hire
- Board Certification or Board Eligibility in Anesthesiology, Emergency Medicine, Family Medicine, Internal Medicine, Obstetrics / Gynecology, Neurology, Pediatrics, Physical Medicine / Rehabilitation, Psychiatry, Radiology, or General Surgery by an American Board of Medical Specialties (ABMS) recognized board required upon hire
- Board Certification or Board Eligibility in Hospice and Palliative Medicine by an ABMS Board, or certification by the American Board of Hospice and Palliative Medicine required upon hire
- Drug Enforcement Agency Certificate required upon hire
- Prescription Monitoring Program required upon hire
- National Provider Identifier required upon hire
