About the Role:

Galileo is looking for a primary care Nurse Practitioner/Physician Assistant with a strong desire to initiate relationships and engage patients managing complex medical conditions. Our ideal candidate would have a passion for improving health outcomes in those who need the most support. Unlike traditional medical practices, at Galileo, you'll be given the time and tools necessary to perform a comprehensive Initial physical exam, intake, and assessment. Then, have the opportunity to collaborate with an excellent team of multi-disciplinary experts at the frontier of modern medicine who will manage the patient's ongoing medical care.

Here’s what you’ll do:

  • Establish patient relationships through Initial Visits
  • Travel to patients to meet them in the comfort of their homes or in our state-of-the-art, fully equipped Medical Mercedes Mobile Sprinter Van
  • Provide Initial Visits, Annual Wellness Visits, and Establish-Care visits with (Medicare and Medicaid patients) 
  • Transition primary care patients to their continuity provider who will manage their preventive health needs, chronic disease challenges, and transitions of care
  • Collaborate and partner across care team members, including social work, behavioral health, and clinical subspecialists
  • Participate in occasional urgent care and triage visits to support patients and care team members

Patient Advocacy

  • Identify and help to address any financial, social, environmental, or functional barriers to care or better health

Population Health

  • Participate in improving population health outcomes while working with other team members to drive ongoing improvements in our care model and decrease inefficiencies 

About You:

You are a primary care provider with a strong passion for conducting initial patient visits and engaging patients and their families.  You are enthusiastic and outgoing, comfortable with complex clinical and social needs and are eager to interact directly with patients in their homes and community environment.  You are driven to work with a community of providers who are empathetic, finding ways to support patient care and make meaningful and real impacts in their lives.

We would love to hear from you if you have the following or equivalent experience:

  • 2+ years of clinical experience in complex care, chronic care, and primary care
  • Innovative and creative leadership qualities
  • Proven experience with successful behavior modification
  • Comfort with basic procedural skills
  • Ability to travel across assigned territory with reliable access to personal transportation
  • Completed an accredited [NP/PA] program with certification to practice
  • Active licensure in Alabama or ability to obtain license by start date with Galileo
  • For the safety of our patients and staff, employees in this role must be fully vaccinated with all necessary vaccinations. 

Travel Requirements: This is an in-person role supporting our patients located in Birmingham, AL) and surrounding counties. As part of this role, you will be providing direct patient care in patients’ homes, from a mobile clinic or virtually.

You’ll need to have access to personal, reliable transportation. We provide reimbursement for commuting costs to and from patient visits.

Physical Requirements: The employee must be able to meet the following requirements with or without reasonable accommodation(s):

  • This position requires the ability to stand, sit, and walk for extended periods of time
  • Ability to lift and carry objects weighing up to 20 pounds
  • The job involves frequent bending, reaching, and kneeling to perform various tasks

Compensation:

$148,000-$160,000
