Job Summary
Compiles, processes and maintains medical records of hospital and clinic patients in a manner consistent with medical, administrative, ethical, legal, and regulatory requirements of the health care system. Processes, maintains, compiles and reports patient information in accordance with health requirements and standards.
Job Details
Responsibilities:
- Performs medical record handling functions, including, but not limited to, record reconciliation, chart retrieval, filing of paper charts and loose documents and tracking in internal and external computer systems.
- Maintains and operates a variety of health record databases and HIM related applications, to collect, classify and analyze information.
- Uses computer programs to tabulate and analyze data to improve patient care, better control cost, provide documentation for use in legal actions or use in research studies.
- May process birth certificates and related documents and enter into IPHIS system per facility HIM process, if assigned.
- Prepares electronic or paper copies of medical charts ensuring compliance with HIPAA regulations and release information to persons and agencies according to regulations.
- May process Stat ROI requests and Third Party Requests.
- May process mailed informational and educational material to patients prior to scheduled appointments.
- Performs quality audits of RAC or third party requests as assigned.
- May perform medical record deficiency analysis and related medical staff suspension process, if assigned.
- Assists providers in Epic and/or other electronic system usage relative to medical record deficiency completion.
- Prepares reports related to medical record deficiency analysis, scanning process and other operational processes, as assigned.
- Performs scan preparation/scan/index and re-index of medical records, as assigned.
- May perform various administrative duties, including, but not limited to, answering/forwarding telephone calls, faxing, typing, sorting mail and preparation of correspondence.
- Participates in Weekend and Holiday coverage per specific HIM site schedule.
- May investigate potential duplicate medical record numbers and perform related merge functions.
- Other duties as assigned.
Education:
- High School Diploma or GED required.
- Associate or Bachelor Degree in an Allied Health program is preferred.
Certifications:
Complexity of Work:
- Requires attention to detail, ability to follow directions and the ability to work with minimal supervision.
- Must be able to work in a stressful environment.
Work Experience:
- A minimum of one year of health information management experience, including experience dealing with the public in person and/or via the phone is required.
- Associate or Bachelor Degree may offset one year of required experience.
- Medical terminology with intermediate PC skills (data entry, word processing, and spreadsheet) preferred.
Physical Requirements:
- Ability to perform work in a stationary position for extended periods.
- Ability to travel throughout the hospital system.
- Ability to work with physical records, such as retrieving and filing them.
- Ability to operate a computer and other office equipment.
- Ability to communicate and exchange accurate information.
- In some locations, ability to move up to 25 lbs.
