Scope of Work

Responsible for providing quality education, follow-up and emotional support to the population and their familiesCase manages the population throughout the inpatient and outpatient settings and measures outcomes to maximize continuity of care between the settings. Manages projects and programs related to clinical management, performing quality improvement activities to measure program processes and outcomes.

Qualifications

  • Required Bachelor's Degree Nursing
  • Preferred Master's Degree Nursing
  • 5 years of relevant experience previous nursing or CDE clinical experience Required
  • Prior teaching with patients of all ages Preferred
  • LIC-Registered Nurse (RN) - STATE_MI State of Michigan Upon Hire required
  • CRT-Basic Life Support (BLS) - AHA American Heart Association Medical Group, Grand Rapids Hospitals, Spectrum Health System 90 Days required Or
  • CRT-Basic Life Support (BLS) - ARC American Red Cross Medical Group, Grand Rapids Hospitals, Spectrum Health System 90 Days required
  • CRT-Breast Patient Navigator - NCBC National Consortium of Breast Centers Oncology Breast NN 1 Year required
  • CRT-Oncology Certified Nurse - ONCC Oncology Nursing Certification Corporation WITHIN 1 YEAR Oncology departments in Gerber, Grand Rapids, United, and Zeeland 1 Year required
  • CRT-Adv Cardiovascular Life Support (ACLS) - AHA American Heart Association SHMG 120 Days required Or
  • CRT-Pediatric Adv Life Support (PALS) - AHA American Heart Association SHMG 120 Days required
  • CRT-Diabetes Educator (CDE) - NCBDE National Certification Board for Diabetes Educators Pediatrics in Medical Group and Spectrum Health Hospitals Upon Hire preferred
