Job Description:
Scope of Position: The Survivorship Coordinator will lead the development, implementation, and ongoing management of a comprehensive cancer survivorship program, beginning within our breast clinic and eventually expanding across the Mary Bird Perkins Cancer Center network. This individual will serve as a key facilitator between clinical teams, patients, and community resources—ensuring that survivorship care plans are meaningful, evidence-based, and scalable across our enterprise.

QUALIFICATIONS:
Degree in Nursing, Social Work, Public Health, or a related field required
Minimum 3 years of experience in oncology or chronic care coordination; survivorship experience strongly preferred.
Strong project management, research, and organizational skills. Analytical skills to gather and summarize information for planning & development
Excellent communication and interpersonal skills, with the ability to work across departments and levels.
Proficient in EMR systems and data reporting.
Ability to multi-task, prioritize, think independently, take initiative, meet deadlines
Familiarity with ASCO, NCCN, or ACS survivorship guidelines.
Experience with program development and quality improvement.
Knowledge of CoC standards and accreditation processes.
