Position Highlights:

• Location: Northwest Community Hospital - Arlington Heights, IL
• Full Time, 40 hours/week
• Hours: 8:00am-4:30pm
• Rotating weekends

What you will need:

• Master's Degree from an accredited school of Social Work
• Licensure to practice social work in Illinois: LSW/LCSW or LSW/LCSW eligible. Must obtain minimum of LSW within 90 days of hire
• Minimum of two years recent clinical experience in the social work filed in an acute care setting; mental health setting and/or medical social work experience preferred
• Current CPR certification a plus

General Summary:

Reporting to the Director, Clinical Care Coordination, works with all healthcare team members to promote the effective utilization of services and assists with the coordination of care for neonate, pediatric, adolescent, adult, and geriatric. Contributes to the team’s effectiveness by reviewing and implementing plans of care, identifying barriers to effective utilization of resources, and appraising patient’s psychosocial, financial, and clinical needs. Functions as a liaison between patients, the hospital, and the community and ensures that patient’s continuity of care needs are met. Identifies community resources, coordinates financial and clinical aspects of health care benefit plans, and counsels patients on health care and treatment options. Facilitates and executes complex discharge planning for patients.
