About the Department

This Serious Illness Care Navigator will be focused on people with advanced heart failure navigating complex health journeys. This role is the compassionate bridge be working closely with the clinical team at Heart OPtimization and Evaluation Clinic (HOPE) to help people and their families understand their health and goals based on what matters most to them. The SICN role will:

  • empower patients and families to understand and make informed health decisions based on what matters most to them.
  • ensure personalized care by connecting patients and families to services that align with their goals and values.
  • Collaborate with experts by working hand-in-hand with the clinical team to ensure person centered care for every person.

Work of scope

The Serious Illness Care Navigator (SICN) will assess the patient's individual needs and develop a plan of care to meet those needs. This role includes providing education and support to patients, families, staff and caregivers. Demonstrates expertise, current knowledge in care management of a caseload of patients with high complexity and seeks to improve patient, family and health systems /community outcomes through the application of education concepts/skills and planning for medical crisis and end
of life in a management care environment. SICNs will have visits with patients and families while hospitalized, in post-acute (skilled nursing facilities) and completed home visits to manage patient goals and needs for their health care.

  • Knowledge and ability to ask appropriate clinical questions, and illicit patient information to determine appropriate action(s).
  • Improve patient experience by connecting patient and family with appropriate resources, services, and community resources
    that align with what matters most to them.
  • Priorities include improving paitent and families understanding of health status and eliciting what matters most in the context of their heath.
  • Access to multiple network clinical information systems to gather and verify patient demographic and clinical information.
  • Collaborate with all care providers to assist in the provision of quality, cost effective patient care.
  • Provide individualized support, education and information, helping to make care seamless, continuous and comprehensive.
  • Serve as a liaison between the patient and their family and the various hospitals, physicians offices, community programs and post-acute facilities with whom they interact.
  • Responsible for patients within the inpatient, outpatient, and post acute (skilled nursing facilities).

Qualifications

  • Required Bachelor's Degree Nursing
  • 3 years of relevant experience supporting individuals with complex medical conditions and social determinates of health Required
  • Registered Nurse (RN) - State of Michigan Upon Hire required
  • Basic Life Support (BLS) - AHA American Heart Association 90 Days required Or
  • Basic Life Support (BLS) - ARC American Red Cross 90 Days required
