Job Description:

Role title:  Clinical Nurse Consultant (CNC) Grade 2 – Renal Supportive Care

Role type:  Permanent part time (16 hours per week)

Department and location:  Renal Ambulatory Care, St Vincent’s Hospital, Darlinghurst

Salary:  $135,826.04 - $138,534.10 (pro-rata) + super.

About the role

  • Experienced nurse to develop a service to provide patient-centred care for renal patients who are on a Supportive Care Plan and will include: developing initiatives, setting up clinics or outreach services and helping patients to manage severe persistent symptoms.

  • Promote and develop the Renal Palliative Care nursing service, fostering excellence in clinical, educational, research and professional leadership.

  • Planning and coordinating renal memorial services for families and developing supportive care pathways and a home visiting program.

  • The role requires a high level of self-direction and autonomy, and the applicant will need to engender strong engagement between the Renal and Palliative Care services.

About you

  • Currently registered as a Registered Nurse (Division 1) with the Australian Health Professionals Regulation Agency.

  • Approved post registration nursing/midwifery qualifications relevant to the field in which he/she/they are appointed, or such other qualifications or experience deemed appropriate by St Vincent’s Hospital, Sydney.

  • At least 5 years full time equivalent post registration experience.

  • Demonstrated high level effective communication and interpersonal skills.

  • Demonstrated effective time management skills and ability to prioritise competing workloads within a complex environment.
