  • Job Summary:

  • The Manager of Virtual Care & Community Engagement works with the AVP for execution of the day-to-day strategic
    plan for virtual care and community engagement activities and initiatives. This position will manage operations related
    to population health/value-based programs and transitional care programs.


    Core Responsibilities and Essential Functions:


    Manages the Population Health team daily tasks and roles by ensuring execution of the strategy plan
    related to financial, quality and regulatory program KPIs are being achieved.
    Oversees the Virtual Care at Home and Hospital at Home programs in collaboration with the Lead Nurse and
    Medical Director of the program by ensuring compliance with AUHS/AUMC policies and standard operating
    processes; implements new clinical conditions and SOPs when needed.
    Daily oversight of the Mobile Outreach Fleet including scheduling, clinical and operational processes and
    setup, and relationship management with various vendors/sites and community resources.
    Coordinates outreach activities with the AVP of Virtual Care & Community Engagement and the Service
    Chief of Virtual Care for the Rural Virtual Care Network including site visits, education sessions/training on
    the virtual care program, and serving as a liasion for concerns/issues from the network to AUHS.
    Manages the daily operations of the Chronic Care Management (CCM), Transitional Care Management
    (TCM) and other care coordination activities within the Population Health Department.
    Performs other duties as assigned
    Complies with all Wellstar Health System policies, standards of work, and code of conduct.


    Required Minimum Education:

    • Bachelors Health Science or Bachelors Nursing or Masters Health Science-Preferred or Masters Nursing-Preferred
     

    Required Minimum License(s) and Certification(s):


    All certifications are required upon hire unless otherwise stated.
       

      Additional License(s) and Certification(s):


      If RN, current Registered Nursing license
      (Georgia) or qualifying multi-state compact
      license Upon Hire Required
      RN-NP Upon Hire Preferred


      Required Minimum Experience:


      Minimum 3 years Community Outreach Programs, Population Health/Value-Based Programs Required or
      Minimum 5 years Healthcare experience related to operations or direct clinical experience Preferred
