RN Clinical Team Specialist | Home Health | OASIS | Care Coordination
Are you an experienced Home Health RN with strong OASIS documentation, care coordination, and Homecare Homebase (HCHB) experience? Ready to step into a role where your clinical expertise helps guide patient care and support your team?

What You Will Do
• Review and approve home health plans of care
• Conduct OASIS assessments and OASIS reviews
• Monitor documentation in Homecare Homebase (HCHB)
• Coordinate patient care, referrals, and clinical documentation
• Ensure visits and documentation meet Medicare, state, and federal compliance requirements
• Support quality assurance, performance improvement (QAPI), and home health regulatory standards
What You Will Bring
• Active Registered Nurse (RN) license
• Minimum 2 years home health RN experience
• Strong knowledge of OASIS documentation, home health compliance, and care planning
• Experience with Homecare Homebase (HCHB) preferred
• Proficiency with electronic medical records (EMR) systems and Microsoft Office
