Summary:

The Telehealth Nurse (RN) as an active member in the collaborative patient care team advocates for patients and supports bedside staff, facilitates admissions, discharges and education, aligns practice with safety and quality, and engages in professional development.

Responsibilities:

Delivers, manages, and coordinates nursing services virtually, as part of the care team, with bedside team through direct observation via video monitoring to provide care.Rounds virtually evaluating and documenting patient response to nursing interventions and alters the plan in collaboration with the bedside nurse.Develops and implements individual plans of care in collaboration with other members of the healthcare team in accordance with the established guidelines and standards of nursing care.Facilities the use of remote sensor technology such as, but not limited to, Hospital at Home type equipment for monitoring blood pressure, weight, glycemic testing, etc.Performs assessments on assigned patients in collaboration with bedside nurses to develop admission and discharge plan.Participates in safe medication administration by performing dual signoff and guidance to the bedside nurses (when available and appropriate).Collaborates with all members of the health care team including physicians, ancillary members and other clinical staff when planning patient care, scheduling, assessing, and facilitating remove exams; documents accordingly for follow-up care.Supports patients and families with spiritual/cultural needs through teaching and learning from admission to discharge and post-acute care referrals. Documents learning level and readiness in the education record of the EHR.

The incumbent must be able to lead Required Work Experience: Minimum of three (3) years of intermediate/step down level of care, ED or critical care experience and/or several years of virtual nursing experience required.
