Overview

Manages the monitoring systems used for the evaluation and tracking of the wound management program and initiatives for all VNS Health provider services. Recommends and implements new/revised technologic systems as needed. Plans, manages, and evaluates clinical orientation and ongoing education and training programs for staff for wound and ostomy management programs across VNS Health. Collaborates with regional/program leadership in the delivery of new hire and ongoing clinical education and competency development. Partners with peers across the enterprise to develop clinical standards, guidelines, relevant statutes, rules and regulations. Facilitates consistency of dissemination of standardized clinical education policies, procedures and programs.

Compensation Range:$109,900.00 - $146,500.00 Annual

What You Will Do

  • Manages and delivers wound care specific clinical education programs designed to foster high quality care by implementing evidence-based practice, performance improvement activities, and professional staff development.
  • Supports the day-to-day operations of the wound management software application.
  • Ensures clinical education programs address patient-centered needs. Identifies and communicates protocols and standards of care for a culturally and demographic diverse patient population; provides education intervention guidelines based on these population health needs.
  • Manages the implementation of clinical staff development programs such as orientation, in-services, training, and other learning activities. Ensures that the development and delivery of the program
  • Reviews program-specific educational program outcomes and program evaluations to determine areas for improvement and makes recommendations for change and new programs for clinical education as appropriate.
  • Supports the integration of evidence-based wound and ostomy care practices into protocols/policies, consultation strategies, staff development plans, continuous quality improvement initiatives, and research.
  • Evaluates clinical outcomes data and analyze practice patterns to promote organizational responses that meet the needs of patients with specific complex and chronic illnesses.
  • Develops policies, standards, and protocols for applying evidence-based practice and evaluates their impact on care management outcomes.
  • Collaborates with program leadership to identify, develop, implement, and evaluate practice improvement plans.
  • Evaluates staff practice patterns on an individual and team level to identify gaps and improve communication skills and clinical practice related to wound care.
  • Performs written, telephonic, and in-person clinical consultations as needed.
  • Participates in creating and delivering presentations on project status and outcomes to management.
  • Participates in special projects and performs other duties as assigned.

Qualifications

Licenses and Certifications:

  • Certified as a Wound Ostomy Continence Nurse required
  • Current License and registration to practice as a Registered Professional Nurse in NYS required


Education:

  • Bachelor's Degree of Science, Science in Nursing, Science in Education or Science in Business Administration required
  • Master's Degree of Science in Nursing, Science in Education, Science in Business Administration preferred


Work Experience:

  • Minimum of five years progressive experience in health care required
  • Conversant in data interpretation and presentation required
  • Proficient with personal computers, including Windows, Excel, Word, and PowerPoint required
  • Knowledge of Project Management Applications, and other related databases preferred
