Job Summary:
The Cardiovascular Disease Coordinators will serve as a resource for patients, physicians, and staff, performing in the
role of education, quality improvement champion and consultant for patients with cardiovascular disease. This position
is able to function independently and demonstrate self-motivation. Reinforce adherence with best practice guidelines.
Work closely with the AU Health faculty and management team to analyze data and formulate Quality Improvement
projects based on findings.
Core Responsibilities and Essential Functions:
Clinical - Demonstrates understanding of CV Disease and the care of this patient population. Ensures a process
of identifying these patients in the facility. Responsible for organizing services and systems care of these
patients. Works collaboratively with clinical staff to ensure evidenced-based practice guidelines are utilized
to optimize patient care. Actively participates in the development and implementation of standardized care.
Performance Improvement - Develops a continuous process of monitoring, assessment, and management
that is supported by the registry database. Coordinates daily data processing and monitoring the
effectiveness of established evidenced-based practices.
Education - Is seen as a clinical expert in the specialty by obtaining certification in the specialty. Provides staff
education as needed. Able to collaborate and work across external organizations and internal
multidisciplinary departments. Develops a program to engage referring facilities to improve outcomes for
patients. Assists with moving patients through the complex healthcare transition as necessary.
Registry and Accreditation - Gathers key data elements, assuring the integrity of the data, consistently
utilizing national standardized definitions and uses the data to improve the care of the cardiovascular patient.
Communicates effectively with the primary care team, including physicians and staff to identify and resolve
variances in care. Evaluates outcomes against benchmarking data.
Community Outreach -Engages patients and the community by developing educational materials/offerings
on cardiovascular disease. Develops a process for training of external organizations to improve the transition
of care.
Performs other duties as assigned

All certifications are required upon hire unless otherwise stated.
- BLS-I - Basic Life Support - Instructor or BLS - Basic Life Support
- RN-COMPACT - RN - Multi-state Compact or RN - Reg Nurse (Single State)
Additional License(s) and Certification(s):
Required Minimum Experience:
Minimum 3 years Cardiovascular acute care nursing Required or
Minimum 5 years Cardiovascular acute care nursing Preferred and
Required Minimum Skills:
Proficient with Microsoft office: Powerpoint, Excel, Access, and Word High
