Position Summary


Description:


Aetna’s Healthy Aging Support Program (HASP) is a short term, self-management education program.  The HASP RN is responsible for telephonically assessing, planning and one on one coaching to build members’ confidence and ability to manage their health.  This role evaluates the medical needs of the member to facilitate the member’s overall wellness. The HASP RN will work one on one with Medicare members on specific content addressing common health concerns for members with chronic conditions, providing education and resources, and effectively utilizing community, health care, and plan benefit resources.

Fundamental Components:

  • Clinically assess all HASP enrolled members to determine their overall health status, coordinating with Medicare Care Management when needs supersede HASP program participation requirements
  • Educate members on an array of topics such as Managing Medicines, Nutrition, Safety, Benefits/ Resources, Mindfulness and Working with Care Team and empower participation of the group coaching participants
  • Assist the member in actively participating with their health care providers and Aetna Care Management teams regarding decisions that impact their health and well-being
  • Document all member interaction into the appropriate member record(s)
  • Demonstrate empathy for situations faced by our members and their caregivers
  • Troubleshoot unavailability of resources, securing support as needed to close member gaps
  • Follow program workflows consistently, following up with members to review their personal action plan and determine if any additional referrals are needed (e.g., Care Management, Behavioral Health, etc)


Required Qualifications

  • 5 years clinical practice experience
  • Candidate must support 8:00 AM - 5:00 PM Mountain Time or 7:00 AM - 4:00 PM Pacific Time
  • Active and unrestricted Compact RN licensure


Preferred Qualifications

  • Managed Care/Medicare/Medicaid experience preferred
  • Bilingual preferred - Spanish
  • Effective communications, Organizational and interpersonal skills
  • Ability to work independently
  • Efficient and effective computer skills including navigating multiple systems and keyboarding


Education

  • Associates Degree or Nursing Diploma Required
  • Bachelors Degree Preferred

Pay Range

The typical pay range for this role is:

$54,095.60 - $116,760.80

This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls.  The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors.  This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above.
