Job Description Summary:
This position provides hygiene, comfort, and safety needs to assigned patients of all ages, under the direct supervision of a RN. He/She may conduct psycho educational and symptom management patient groups.Minimum Qualifications:

SPECIALIZED KNOWLEDGE
Patient Care Tech training or equivalent training with proof of completion. six months Experience as a clinical assistant or patient care assistant. Must have current Basic Life support (BLS) and Crisis Prevention Intervention (CPI) within the first 3 0 days of hire and must remain current throughout employment. High degree of accuracy and attention to detail. Emotionally mature, uses sound judgment; Ability to make decisions calmly and quickly. Agility to intervene with aggressive patients. Knowledge of Microsoft Office Products. Demonstrated ability to organize/coordinate activities/groups. 1-2 yrs. previous Experience in health care crisis intervention program

Under Supervision of a Registered Nurse, Assists in Providing Care as Appropriate 1. Establishes caring and supportive relationship with patient and family. 2. Helps patient use problem-solving and coping skills appropriately. 3. Serves as role model for healthy behaviors and effective communication skills. 4. Documents observations and patient data in accordance with hospital policy. 5. Orients patients and families to unit, program goals, and guidelines.

Conducts Psycho Educational and Symptom Management Patient Groups, Under Direction of a Registered Nurse 1. Organizes, plans, directs psycho educational, symptom management, coping skills groups for patients, and documents pertinent information. May direct community meeting. 2. Adheres to group schedule. 3. Maintains current documentation in patient medical record. 4. Refers questions or situations requiring professional judgment to registered nurse.

Assists in Performing Hygiene, Comfort, and Safety Needs of Patients Under Direction of, and as Assigned by RN 1. Assists patients with personal care as needed, indicated, or assigned, including, but not limited to: bathing, oral hygiene, care of hair and skin, shaving. 2. Assists patients with elimination as needed, indicated, or assigned. 3. Assists with and performs treatment procedures under supervision of an RN, within prescribed limits and hospital policy, e. g. , measurement and recording of intake and output, collection of specimens, application of ice bags or non-sterile warm packs. 4. Serves trays, feeds patients, records intake and calorie counts. 5. Checks and records vital signs accurately. 6. Transports patients safely.

Assesses Environment for Potential Hazards and Initiates Safety Measures 1. Conducts and documents regular patient safety checks; reports concerns and significant changes to RN. 2. Maintains safe and hygienic environment, identifying and reporting safety hazards, keeps hazardous material in proper and safe place. 3. Checks in patient belongings; checks for and secures contraband. 4. Follows infection control procedures and universal precautions in providing patient care.

Organizes Work to Complete Assignments in a Timely Manner 1. Follows through with directions and assignments in a timely manner.
