Your Impact in This Role

• Conduct in-home evaluations for speech, language, cognition, voice, and swallowing

• Develop and implement individualized treatment plans per physician orders

• Educate patients and caregivers to support progress between visits

• Collaborate with nurses, therapists, and physicians to ensure coordinated care

• Complete timely, accurate documentation

• Promote patient safety and help reduce avoidable hospitalizations

What You’ll Bring

• Master’s degree in Speech-Language Pathology from an accredited program

• Active state SLP license (or Educational Limited License with supervision)

• Prior SLP experience preferred; home health or hospice experience a plus

• Strong time management and independent decision-making skills

• Compassionate, patient-centered approach

• Reliable transportation and valid driver’s license
