Job Description:

Supportive interdisciplinary team and clinical leadership

Conduct in-home evaluations for speech, language, cognition, voice, and swallowing
Develop and implement individualized treatment plans per physician orders
Educate patients and caregivers to support progress between visits
Collaborate with nurses, therapists, and physicians to ensure coordinated care
Complete timely, accurate documentation
Promote patient safety and help reduce avoidable hospitalizations
Master’s degree in Speech-Language Pathology from an accredited program
Active state SLP license (or Educational Limited License with supervision)
Prior SLP experience preferred; home health or hospice experience a plus
Strong time management and independent decision-making skills
Compassionate, patient-centered approach
Reliable transportation and valid driver’s license
