PURPOSE AND SCOPE:

Provide high quality patient care within the scope of training and education. Assess patient care plans and provides comprehensive patient care independently or in autonomous collaboration with other health professionals. Cooperates with other health care personnel to coordinate patient care and establish mutual goals. Relies on experience and judgment to plan and accomplish goals. Demonstrate and fosters a commitment to patient satisfaction and teamwork.  Supervision of Nurse Practitioners and Physician Assistants (Advanced Practitioners), training and clear direction to the clinical team, helping develop and communicate FMCNA's vision and expectations of the group as well as enforce policies and procedures.

PRINCIPAL DUTIES AND RESPONSIBILITIES:

CLINICAL CARE RESPONSIBILITIES:

  • Collects data to support patient assessment including taking vital signs, social history, chief complaint and current concerns
  • Alerts physician to any patient complaints, especially those that are, life threatening
  • Supports clinical functions to prepare patient for examination, treatment or procedure
  • Calls pharmacy refills and patient callbacks as directed by office protocol and as directed by a physician
  • Document all calls, correspondence and encounters with patient within established guidelines
  • Utilizing the nursing process, assesses the patient's condition, learning needs in regard to their health care, provides education as directed by regarding treatment, special diets, medication, follow-up diagnostic tests and healthy lifestyle information
  • Round on patients in a dialysis setting on designated schedule
  • Provide care for hospitalized patients: Assess, diagnose and treat patients in the acute hospital setting with the assistance and under the direction of the supervising physician.
  • Administers prescribed medications and treatments as directed by physician in accordance with the nursing standards.
  • Provide CKD care: Assess, diagnose, and treat patients in the CKD setting under the direction and supervision of the physician when possible and independently if needed and states nursing scope of practice allows
  • Performs procedures and treatments, within the scope of their training and experience, as ordered by the physician following established protocols and appropriately documents outcomes in the patient record
  • Triage emergency and non-emergency phone calls with appropriate destinations (i.e.: Emergency Department etc), documents according to protocols
  • Participate and lead the development of clinical protocols in obtaining, preparation and transport of specimens for diagnostic tests as ordered by the physician and using universal precaution guidelines
  • Observes, records and reports patient's condition and reaction to drugs and treatments to physician
  • Assists in the coordination of patient transfers to hospital (direct admissions)
  • Subscribe to and read professional periodicals and journals to stay up to date on clinical practice
  • Follows established protocols to report and document diagnostic tests as directed by the physician, within the scope of training and experience
  • Reviews patient records and other pertinent information and takes appropriate action within nursing standards and office protocols
  • Documents and reports clinical information or complaints taken over the phone to the physician in an appropriate manner within established timeframes
  • Works on problems of diverse scope where analysis of data requires evaluation of identifiable factors. Demonstrates good judgment in selecting methods and techniques for obtaining solutions.
  • May escalate issues to supervisor/manager for resolution, as deemed necessary.

SUPERVISOR RESPONSIBILITIES:

  • Review, comply and enforce the Code of Business Conduct and all applicable company policies and procedures, local, state and federal laws and regulations.
  • Knowledge of healthcare quality Assurance/Quality Control program and Federal and State regulations pertaining to quality and utilization management.
  • Familiarity with evidence-based practice and KDIGO/KDOQI best practice guidelines and algorithms.
  • Provide leadership, training and clear direction to the clinical team, helping develop and communicate FMCNA's vision and expectations of the group as well as enforce policies and procedures. Ensure that these expectations are met.
  • Responsible for overseeing Advanced Practitioners daily clinical operations, schedules and executing clinical protocols and best practice guidelines. Sustain team effort and encourage maximum individual contribution to ensure clinical efficiency, quality, service, and cost-effective management of resources.
  • Develop and maintain nephrology related healthcare teaching protocols for related staff.
  • Conduct and attend related staff meetings and communicate related clinical protocols, policies and procedures.
  • Work to resolve issues related to clinical operations and quality care with operations management team in a timely manner.
  • Conduct Advanced Practitioner staff meeting at minimum on a quarterly basis
  • Conduct Quarterly Education day with Advanced Practitioner clinical team
  • Conducts related periodic audits of staff, HIPPA compliance to ensure safe and quality patient care. Responsible for monitoring the staff with respect to patient's rights, privacy and confidentiality at all times. Communicates audit findings, monthly stats, and monthly reports to related parties in a timely manner.
  • Assist and support senior management team in all aspects of company growth specifically through strategic and operational planning guidance and by ensuring that required infrastructure and support services are in place.
  • Assist with various projects as assigned.
  • Other duties as assigned.

    • Infrequent (1-11%) crawling or crouching.
    • Must be able to lift up and pull/push up to 25 pounds.
  • Travel may be required and to all related hospitals and outpatient facilities.

 

SUPERVISION:

  • Responsible for the direct supervision of the Advanced Practitioner staff

 

EDUCATION:

  • Master's Degree required
  • Educated and trained at university graduate level, nationally Board Certified in specialty area based on NP/PA licensure and state authority for advanced practice.
  • Nurse Practitioner/Physician Assistant License
  • CPR Certified

 

EXPERIENCE AND REQUIRED SKILLS:  

  • 2 - 3 years nephrology nurse practitioner/physician assistant experience or acute care setting required
  • 1 -- 3 years nursing supervisor experience preferred
  • Demonstrated proficiency in computer skills, such as EHR, data entry, Windows, Microsoft Word, Excel, E-Mail and Internet access.
  • Knowledge of healthcare quality Assurance/Quality Control program and Federal and State regulations pertaining to quality and utilization management.
  • Familiarity with evidence-based practice and KDIGO/KDOQI best practice guidelines and algorithms.
  • Innovative, creative, excellent people skills, organized with excellent time management.
  • Ability to demonstrate excellence in verbal and written communication and assessment skills, as well as analytical critical thinking.
  • Ability to calculate figures and amounts such as discounts, interest, commissions, proportions, percentages, area, circumference, and volume. Ability to apply concepts of basic algebra and geometry.
  • Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists. Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.
  • Strong problem-solving skills and ability to organize and prioritize work and multitask. Demonstrated ability to work independently and self-directed.
