Job Description:

Role title:  Nurse Practitioner (Level 1 - 4)

Role type:  Permanent part time (24 hours per week)

Department and location:  HIV Outpatient Unit - Immunology B Ambulatory Care (IBAC), St Vincent’s Hospital, Darlinghurst

Salary:  $143,851.10 - $154,025.92 (pro-rata) + super.

About the role

  • Comprehensive clinical assessment, requesting diagnostic investigations, management of care for patients/clients, prescribing medicines/therapies, including direct referral of patients to and from other health care professionals.

  • Educated and authorised to function autonomously and collaboratively in an advanced and extended clinical role.

  • Using a case management model and in collaboration with the CNS for HIV complex care and the HIV social work team, provide support and care for patients living with HIV with complex bio-psychosocial needs promoting medication adherence, engagement in care, harm minimization and service(s) navigation.

  • Provide leadership that facilitates the ongoing development of clinical practice by identifying and participating in research, quality, audit and evaluation opportunities, in the provision and development of educational programs, clinical services planning and development of clinical practice standards.

  • Senior nursing clinician and resource person for the service and works closely with clinicians and management in the provision of care.

  • Manage and cares for patients, families and significant others through utilising models of care that are appropriate, timely and of a high quality which are governed by authorised guidelines/polices.

About you

  • Currently registered as a registered nurse with the Australian Health Professionals Regulation Agency (APHRA) and endorsed by the Nursing and Midwifery Board of Australia to practice as a nurse practitioner.

  • Demonstrated high level leadership within the workplace and across external agencies including the ability to form productive working relationships through highly developed communication, negotiation, facilitation, and conflict resolution skills.

  • Extensive experience in immunology B and chronic disease management.

  • Comprehensive understanding of clinical governance, clinical practice development and current nursing issues.

  • Demonstrated experience in contribution to nursing research, quality improvement initiatives and policy and procedure development and implementation.

  • Advanced understanding of workplace health and safety.
