About the role:

The Associate, Medical Records is responsible for maintaining, organizing, and managing patient medical records in compliance with regulatory standards and organizational policies. This role ensures the accuracy, completeness, and accessibility of medical records for healthcare providers, while adhering to confidentiality and data security regulations. They will support record requests, collaborate with other departments to facilitate the sharing of medical records, and participate in audits and quality assurance reviews. Additionally, the role includes monitoring and improving record-keeping processes to maintain efficiency and compliance with healthcare regulations.

Your contributions are: 

I. MEDICAL RECORDS MANAGEMENT

  • Provides support and response to any medical records inquiry and request from MDs, nurses, and other authorized persons, in a timely manner.
  • Ensures that all chart requests are attended to by adhering to the proper documentation of process requests and proactively reports any record-related issues or concerns to the team lead or department head for proper management of stakeholders.
  • On a daily basis, closely monitor the incoming/outgoing medical charts, making sure these are returned in a timely manner by  coordinating with the borrower and regularly updating documentation (like the borrower's monitoring sheet)
  • Prepares reports or documentation regarding appointment schedules in a timely manner and may update documentation regarding MD's latest consults and list of patients received as needed

II. DATA INTEGRITY AND ACCURACY

  • Accurately input and verify patient information and treatment details in the Electronic Health Records (EHR) and other digital systems.
  • Proactively identify and resolve discrepancies in medical records to ensure data integrity.
  • Prepare reports or documentation related to patient appointments, MD consult lists, or record gaps as needed.
  • Maintains the orderliness of medical records storage and inventory by accurately tagging, labeling, carefully storing these, and promptly updating the documentation required.
  • Ensures that all user credentials pertaining to record digital systems are kept private and report any observed sharing of access from the operations.
  • Works with compliance, risk management, and audit functions, to ensure that security is appropriately designed and updated, based on feedback and assurance results.

III. CONFIDENTIALITY AND COMPLIANCE

  • Adhere to data privacy laws (e.g., HIPAA) and organizational policies on patient information security.
  • Conduct regular audits of medical records to ensure compliance and data accuracy.
  • Ensure the secure management of digital user credentials and report any unauthorized access attempts.

IV. PROCESS IMPROVEMENT AND DIGITIZATION

  • Provide feedback and support for process improvement initiatives, ensuring alignment with organizational goals.
  • Keep updated with industry best practices to recommend enhancements in medical record management processes.
  • Participates in projects that involve records digitization (attending meetings, providing insights e.g. on types of charts) or process improvements, by sharing operational expertise during the duration of the project, adhering to the agreed project deliverables set by any internal/external groups (e.g vendors) and implementing post-project phases to aid in the success of the project.
  • Work with healthcare providers, auditors, and legal teams to facilitate timely retrieval and sharing of medical records.
  • Assist in the preparation of medical records for internal and external reviews, including audits and quality assurance checks.
  • Collaborate with IT and administrative teams on projects that improve record management workflows.

VI. RECORD REQUESTS AND PROCESSING

  • Process and fulfill medical record requests from patients, providers, and authorized entities promptly and efficiently.
  • Verify proper documentation and authorization before releasing any medical information.
  • Maintain accurate logs and reports of all medical record requests and actions taken.
  • Performs general clerical duties including photocopying, faxing, mailing, requesting office supplies, filing documents and archiving

Qualifications

  • Must have at least completed a Bachelor’s degree or graduate of any 4-year course. 

  • At least one (1) year of experience in an IT support or helpdesk role.

  • Preferably with at least one (1) year of experience working in the healthcare industry.

  • Preferably with experience in handling ICD-10-related cases.

  • Experience supporting a healthcare or corporate environment is an advantage.
