Nurse Manager – Home Care 


Join the Cleveland Clinic team, where you will work alongside passionate caregivers and provide patient-first healthcare. Cleveland Clinic currently serves 14 counties in the greater Cleveland and Akron areas. Divided into 4 teams (East1, East2, West and South), each team is then further divided into smaller geographical areas where each caregiver is assigned to a smaller service area within the team.  
 

Cleveland Clinic Home Care is looking for a Nurse Manager to lead and mentor field caregivers in delivering exceptional care in patients’ homes around northeast Ohio. Cleveland Clinic Home Care has a very supportive leadership team and many resources to ensure that all caregivers provide the best care to patients.  
 

A caregiver in this position works 8:00AM – 5:00PM. This is a remote position with occasional travel to Cleveland Clinic’s Business Operations Center and other sites as needed.  
 

A caregiver who excels in this role will:  

  • Manage the practice of professional nursing and delivery of nurse care on an assigned department. 

  • Establish clinical direction for nursing care delivery, provide clinical supervision and maintain quality outcomes. 

  • Collaborate with liaison departments in maintaining patient care standards. 

  • Identify and resolve issues affecting the delivery of patient care services. 

  • Develop quality initiatives and implement policies and procedures to ensure efficient and effective delivery of health services. 

  • Develop and maintain patient care standards through continuous quality improvement. 

  • Maintain Joint Commission and other regulatory standards, including but not limited to, Medicare Conditions of Participation. 

  • Oversee documentation accuracy. 

  • Manage the operation of the department. 

  • Analyze personnel structure and determine staffing needs. 

  • Provide direction and supervision, evaluate performance, conduct selection and termination processes, and develop strategic plans. 

  • Participate in the development of orientation program and hold all caregivers accountable for responsibilities as identified by their job description. 

  • Oversee the orientation of new caregivers.  


Minimum qualifications for the ideal future caregiver include:  

  • Bachelor of science in nursing (BSN) 

  • Current state licensure as a Registered Nurse (RN).  

  • Basic Life Support (BLS) certification.  

  • One year of healthcare management experience.  

  • Valid Ohio driver’s license and proof of automobile insurance coverage with $100,000/$300,000 coverage.  

  • Demonstrated leadership, management, counseling, problem solving, decision making, conflict resolution and group process skills.  


Preferred qualifications for the ideal future caregiver include: 

  • Master of science in nursing (MSN).  

  • Three years of recent clinical experience.  

  • Two years of management experience in a home health setting.  

  • Knowledge of Medicare regulations.  

  • Experience managing and scheduling staff.

Physical Requirements

  • Requires frequent data entry, sitting, standing and walking.
  • Requires corrective vision and hearing to normal range.
  • May require some exposure to communicable diseases or bodily fluids.
  • Light Work – Exerting up to 20 pounds of force occasionally, and/or up to 10 pounds of force frequently, and/or a negligible amount of force constantly (Constantly: activity or condition exists 2/3 or more of the time) to move objects.
