Position: Full-Time M-F 8-5

Key Responsibilities

As the Behavioral Health Clinical Team Manager, you will:

  • Oversee patient care services, ensuring timely, diagnosis-specific home health services.
  • Lead and coordinate care teams, patient assignments, and referrals to maintain optimal care delivery.
  • Supervise the development, implementation, and updates of individualized patient care plans.
  • Approve plans of care, evaluate proposed changes, and ensure alignment with clinical protocols.
  • Facilitate communication among patients, families, physicians, and the care team.
  • Ensure all regulatory guidelines are met and services are started promptly.
  • Integrate input from all relevant healthcare providers into cohesive care plans.

What You Bring

  • Education: Associate's Degree in Nursing (required).
  • Experience:
    • 2+ years of experience as a Registered Nurse in Home Health or Behavioral Health.
    • 1+ year in a supervisory role.
  • Licensure: Current state RN license.
  • Technical Skills:
    • Proficiency with enterprise systems like Workday and HomeCare HomeBase.
    • Strong knowledge of Microsoft Office Suite (Word, Excel, Outlook).
