  • Salary: $74,317- $117,755 p.a. + Super + Salary Packaging

About the Role:

  • Deliver high-quality, evidence-based mental health care to adults experiencing primary homelessness, mental illness and co-occurring needs, using a person-centred, strengths-based, trauma-informed and harm-reduction approach.

  • Conduct comprehensive assessments, provide clinical interventions, care planning, advocacy, referral and care coordination to support improved health and wellbeing outcomes.

  • Lead and coordinate client care across multiple settings, including participation in multidisciplinary reviews, case conferences and collaborative care planning.

  • Build and maintain strong partnerships with clients, their cultural, social and professional supports, and health and community services across multiple Local Health Districts.

  • Support patients to transition to and engage confidently with mainstream and specialist health services.

  • Maintain professional accountability through accurate documentation, reporting, clinical supervision, ongoing professional development, and active participation in service development and quality improvement activities.

About You:

  • Relevant qualifications as a Registered Nurse, Psychologist, Occupational Therapist or Social Worker, with current AHPRA registration (or eligibility for AASW membership), and a current NSW Driver’s Licence.

  • At least five years’ experience providing mental health assessment, treatment, referral and care coordination to people with comorbid health and support needs in a community setting.

  • Demonstrated understanding of and commitment to working with people experiencing homelessness, using trauma-informed, strengths-based and client-centred approaches.

  • Proven ability to coordinate healthcare, take a lead role within multidisciplinary and multi-agency support networks, and contribute to service development and quality improvement.

  • Strong interpersonal, written and verbal communication skills

  • Demonstrated commitment to clinical supervision and ongoing professional development.

  • Well-developed computer literacy, including effective use of email and electronic patient record systems.
