JOB DESCRIPTION:

SUMMARY OF POSITION

The Integrated Care Team coordinates the treatment of behavioral health conditions (mental health & substance use) and physical health conditions; by providing comprehensive care coordination; care management at times of transition; health promotion activities to improve overall wellness and manage chronic health conditions; and monitoring of critical health indicators using data.  The Care Coordinator provides health promotion activities using evidence based practices individually or in groups. The Care Coordinator will also provide screening and assessment to clients interested or enrolled in Integrated Care, ensuring that each individual is met with competent and compassionate care needed to facilitate effective intervention and referral.

ESSENTIAL DUTIES & RESPONSIBILITIES

  • Assists participants and their families/caretakers in developing self-management skills to improve their health by teaching disease management strategies
  • Provides wellness coaching, assisting family/caretakers and participants to learn how to advocate for themselves so they can independently access medical care that is most appropriate to treat their symptoms. 
  • Facilitates wellness and prevention groups using evidenced based and promising practices self-management models, consistently practicing faithfulness to the model
  • Ensures participants understand how to successfully navigate the health care system by providing education and support to them and their family/caregivers
  • Educates consumers using personal health information and provides education to assist them to use the information for  self-management
  • Ensures participant`s care is coordinated by collaborating and actively communicating with service providers and along with the participant develops strategies to remove barriers; allowing them to achieve optimal health status as they engage in the health care
  • Ensures that participants have access to needed services during transitions from different levels of care.
  • Compliance with annual KPI evaluation
  • Regular and reliable attendance is required.
  • Effectively and efficiently obtain all information for accurate documentation in the EHR.
  • Promote a departmental culture of cooperation and collaboration with other Centerstone, departments, community agencies, hospitals, mental health consumers, and the public at large.
  • Assist participants in applying for SSI/SSDI utilizing the SOAR model.

KNOWLEDGE, SKILLS & ABILITIES

  • Effectively communicate via written, verbal, in person and virtual methods
  • Ability to work independently and be part of a team required
  • Ability to think critically and use data to assist participants required
  • Experience in providing wellness coaching, trainings, and education on health promotion
  • Belief in the concept of mental health recovery

QUALIFICATIONS

         

Education Level              

Bachelor’s degree in social work, counseling, psychology, or health related field required.

Years of Experience       

  • Experience in either a physical health care or behavioral health care treatment environment is required.
  • Experience working in a physical health care environment and behavioral health care environment with adults with mental illness is highly desirable,
