Position Summary
This is an exciting opportunity to be an integral part of a state-wide collaborative (MO-PAP) of professionals and community members whose goals, through the creation of a perinatal psychiatric access line, is to support and educate perinatal health providers (OBGYNs, Midwives, and WHNPs) and perinatal families in order to improve access to behavioral health treatment and supportive care in their local communities.
This position's goals are to provide case management, care coordination, resource navigation, relationship building, and community outreach and education for participating providers, patients, and families as part of a multidisciplinary team.
Job Description
Primary Duties & Responsibilities:
- Care Coordination, Case Management and Resource Navigation to providers enrolled in MO-PAP and their patients.
- Work with BHR care manager to effectively coordinate client care with other service providers and provide community resources to providers and patients to assure continuity of care and navigate potential waitlists.
- Collaborate with assigned caseworkers and community supports already established to ensure appropriate placement and linkage upon discharge, to whom the patient can transfer care when needed.
- Assess for other social determinants of health/psychosocial needs of clients and cohabitating family members, and make referrals as needed.
- Follow-up with both patients and providers at specific time points, to ensure needs are being met.
- Create relationships with perinatal health providers statewide: provide psychoeducation and resources around perinatal mental health and enroll into MO-PAP.
- Maintain active listing, and assist with referrals to pharmacological and non-pharmacological resources including mental health care supports, parent support groups, community care management, and other options to attend to other social determinants of health (diapers, rent assistance, IPV resources, etc.).
- Review patient’s electronic medical record and REDCap database to assist assessment and inform patient’s treatment.
- Complete clinical documentation and psychosocial reports, including the family’s social needs that case management can help address (especially any information that psychiatric evaluations may not include).
- Track relevant data points, determined by the funder, in the patient’s medical record and a REDCap database.
- Work within Multidisciplinary treatment team (in person or via Zoom) and attend multidisciplinary state-wide grant planning and program implementation meetings to facilitate and coordinate quality patient care that aligns with the objectives of the grant.
- Attend Division of Child Psychiatry trainings and meetings as appropriate.
- Meet with community perinatal health providers out in the community or via Zoom to provide information about the MO-PAP resource and psychoeducation around definition and causes of and treatment for perinatal mood and anxiety disorders.
- Remain current in research and knowledge of competencies related to perinatal mental health and the case management duties assigned.
- Function well in a team environment while contributing positive support to colleagues and peers.
- Demonstrate willingness to seek/accept direct supervision as well as respect productive feedback from colleagues and others.

Salary Range
$20.57 - $30.84 / Hourly
The salary range reflects base salaries paid for positions in a given job grade across the University. Individual rates within the range will be determined by factors including one's qualifications and performance, equity with others in the department, market rates for positions within the same grade and department budget.
