Summary :
The Ambulatory Care Clinical Pharmacist practices as an integrated team member within a family medicine, internal medicine or specialized clinic. Following Comprehensive Medication Management principles, the ambulatory pharmacist collaboratively manages the medication related outcomes for chronic and complex disease states. Through referrals and practicing within the State of Wisconsin pharmacy regulations, patients are seen for appointments with the pharmacist where the goals are to provide comprehensive medication management services that improve outcomes and provide affordable medication options. The ambulatory pharmacist contributes to the success of the program through the achievement of quality, clinical and reimbursement metrics. The pharmacist additionally contributes to improved prescribing within the clinic through formal education and academic detailing initiatives.
This position is with the Shawano family medicine call group (Shawano primarily along with Clintonville and Tigerton)
Job Description:
KEY ACCOUNTABILITIES:
- Provide pharmaceutical care by practicing within an integrated care team to design, implement and monitor patient-specific medication and monitoring plans that promote safe, evidence-based and cost-effective use of drug therapy.
- Develop and foster collaborative partnerships with providers and clinic staff.
- Build and promote ambulatory pharmacist services within their assigned clinics.
- Improve medication related patient outcomes through collaborative medication management activities.
- Improve medication adherence through individualization of recommendations, education and motivational interviewing.
- Through the delivery of comprehensive medication management services, decrease total cost of care in addition to improving outcomes.
- Work with the entire care team to ensure coordination of care.
- Contribute to the ambulatory pharmacy program’s success by driving progress in defined focus areas.
- Contribute to ThedaCare’s top performance in ACO performance measures specific to pharmacist interventions, such as medication gaps and adherence.
- Improve clinic and system prescribing through defined Academic Detailing initiatives.
- Implement new clinic, patient care or ThedaCare services as requested.
- In an ongoing manner, maintain clinical knowledge required to practice within the program’s Collaborative Practice agreement in addition to other clinical reasons for referral.
QUALIFICATIONS:
- Doctor of Pharmacy or Bachelors in Pharmacy degree from an ACPE accredited school of pharmacy.
- Wisconsin Pharmacist license
- American Heart Association Basic Life Support (BLS)
- Successful completion of an ASHP accredited residency program preferred.
- Pharmacy Board Certification preferred.
- Two years of experience in ambulatory or clinic settings preferred.
- Certified Immunizer
PHYSICAL DEMANDS:
- Ability to move freely (standing, stooping, walking, bending, pushing and pulling) and lift up to a maximum of twenty-five (25) pounds without assistance
- Job classification is exposed to blood borne pathogens (blood or bodily fluids) while performing job duties
WORK ENVIRONMENT
- Climate controlled office setting with daily movement throughout the hospital campus
- Interaction with department members and other healthcare providers
