Position Details:
Schedule: Saturdays and Sundays starting around 7:00 am (hours are flexible)

On an Average Day, you will:
Assess behavioral health patients and their families.
Develop and implement personalized care plans.
Conduct initial and ongoing evaluations of patient needs.
Communicate significant findings and changes to the clinical team and other relevant parties.
Educate patients, families, and staff on behavioral health and symptom management.
Supervise and support behavioral health LPNs.
Conduct Med Administration Visits
Requirements:
Current, unrestricted state RN licensee
1+ years of experience in a clinical care setting as a nurse
Able and willing to travel within branch/office coverage area.
Must have a dependable vehicle, valid driver’s license, and current auto insurance in accordance with state laws.
Must be able and willing to travel 50%
Associates Degree or Bachelors Degree in Nursing is preferred
Previous Home Health or Hospice Experience is preferred
Previous experience with Homecare HomeBase is preferred
