JOB DESCRIPTION:

SUMMARY OF POSITION

The Mental Health Juvenile Justice (MHJJ) Liaison is responsible for assisting in the coordination, development, marketing, organization, and management of assigned program areas.  The MHJJ Liaison will facilitate the identification, screening and referral of at-risk and justice-involved youth who present with mental health concerns.  This is completed through extensive communication with referral sources which may include, but are not limited to schools, truancy boards, probation, Community Mental Health Centers, and Federally Qualified Health Centers.

The MHJJ Liaison assists with logistical planning, overcoming obstacles, addressing stigma and providing support to clients and their families who are navigating the mental health system.  This includes performing continuing assessment of needs and functioning, maintenance of appropriate documentation, assisting with development and implementation of plans consistent with client’s needs, linkages to appropriate services, and advocating on their behalf. The MHJJ Liaison assists Centerstone Leadership in assuring compliance with standards, regulations, and laws.

ESSENTIAL DUTIES & RESPONSIBILITIES

  • Assists clients with making linkages with internal and external resources as outlined in service plan and in coordination with Integrated Healthcare Services. Maintains appropriate contact with referral sources including providing information on client progress, behavior, and needs (with appropriate releases), and maintaining ongoing consultation when appropriate. 
  • Assists client in making discharge plans and provide follow up services as assigned.
  • Completes and submits all required clinical documentation within established timeframes (generally within 24 hours of service provision).  To ensure a complete clinical record, completes Progress Contact Notes for all contacts, canceled/failed appointments and attempted contacts with clients and collaterals within established timeframes.
  • Maintains and submits required program data.

KNOWLEDGE, SKILLS & ABILITIES

  • Effectively communicate via written, verbal, in person and virtual methods.
  • Knowledge of community geographical awareness and of community and agency resources
  • Knowledge of guidelines for confidentiality and professional ethics.
  • Skill in customer service techniques.

QUALIFICATIONS

Education Level

Master’s degree in Social Work, Psychology or other human service related field.  Experience working with youth and families preferred.   

Years of Experience

Two years of related work experience providing services to youth and families in the mental health system preferred.

Certification/Licensure

CPR Certification is required or must be willing to obtain upon employment.

$43,900.00--$61,370.00
