Job Summary: Functioning under the leadership of the Manager of Behavioral Health and located in the Assessment and Resource Center, the Behavioral Health Case Manager serves as the primary liaison between the patient and the identified level of care. The case manager facilitates the discharge planning process in coordination with the Behavioral Health Assessor. The Behavioral Health Case Manager coordinates the sharing of all necessary clinical information to secure inpatient stabilization or outpatient services. The Case Manager documents all contacts as appropriate. In addition, the Case Manager regularly communicates updates on disposition to the Behavioral Health Assessor, physician and team members. The Behavioral Health Case Manager responds to inquiries from the hospital system and surrounding community, engages callers in brief screening to clarify needs, and provides guidance and/or referral to behavioral health resources, as appropriate. The Behavioral Health Case Manager helps to ensure the consistent provision of quality care and supports performance improvement initiatives through departmental audits and data collection. Core Responsibilities and Essential Functions: Facilitation of Patient/Placement/Discharge Planning - Communicates with the BH Assessor after assessment completion in order to facilitate referral to the appropriate inpatient setting or outpatient provider based upon patient age and need for services. - Establish contact and coordination of services with the recommended level of care. - Serve as a liaison between the patient and the determined level of care until disposition is complete: - Complete BHL Pre-Admission Referral Forms electronically - Fax appropriate documentation need to establish disposition - Completes follow up calls with placement resources to ensure timely acceptance and transfer - Coordinate nurse to nurse calls - Coordinate doctor to doctor consultations - Schedule all necessary aftercare appointments with appropriate providers - Verify proof of insurance eligibility and benefits in order to coordinate the next level of care. - Initiate pre-certification for inpatient psychiatric or substance abuse hospitalization - Communicates regular updates in both written and verbal form to the Behavioral Health Assessor, Physician and Nursing Staff to ensure timely discharge to the identified service. Documentation - Documents all contacts initiated with inpatient and outpatient providers to demonstrate linkage to the appropriate level of care. - Documents time and date of accepting physician when a referral for inpatient stabilization has been completed. - Documents details of outpatient referrals to include community provider, date, time and location. - Documents pre-certification information when appropriate. - Documents all necessary clinical information and identified data points to ensure accurate and timely metrics tracking and reporting to key stakeholders. Required Minimum Education: Bachelor's Degree in mental health or other related field. Required Required Minimum License(s) and Certification(s): All certifications are required upon hire unless otherwise stated.
  • Basic Life Support or BLS - Instructor
Additional License(s) and Certification(s): Required Minimum Experience: Experience working within a healthcare system or with a behavioral health provider (internships and volunteer experiences may apply). Required Required Minimum Skills: Ability to communicate effectively both verbally and in writing; capacity to follow directives and provide services that can better meet organizational and patient needs; specific interest in behavioral health and willingness to work at various WellStar sites depending upon need; knowledge of behavioral health practices; basic technological capabilities in Word, and Excel, as well as the ability to utilize a Telemedicine cart for remote assessments.
