Job Summary: The Post Acute Navigation Team Lead is responsible for the leadership, coordination of daily operations and oversight of the Post Acute Navigation Team. The Post Acute Navigation Team Lead must ensure a smooth and efficient transition of patient from the acute care, physician office or other ancillary service areas into the appropriately identified post-acute service. Through this, the Post Acute Navigation Team Lead will align the goals and objectives of Post Acute Navigation with those of Wellstar Health System and each facility. Core Responsibilities and Essential Functions: People * Responsible to provide alignment and consistency between departmental strategic goals and resource assignments. *Effectively escalate issues to payers and other team members to help resolve delays in discharge related to post-acute authorizations. *Monitor for compliance of departmental documentation standard work. *Assist with department schedules and coverage plans (as required). *Participate in the interviewing process with departmental leadership of potential team members, as appropriate. *Serves as primary trainer for all newly hired PAN staff and training PAN team on system workflow updates (Epic, other ancillary systems) *Handles any patient placement escalations (i.e. long LOS and complex placement) *Provide feedback to assist in completing yearly performance evaluations for team. Quality and Patient Safety * Works with facility and PAN team to ensure that patients are directed to appropriate level of post-acute service. * Responsible to educate and train PAN staff and other customers regarding post-acute services criteria and EMR. Customer Service * Works with care coordinators, physicians, PAN team, facility staff, and post-acute service areas to ensure that patients receive an efficient and effective discharge upon notification that post-acute services are desired. * Serves as resource to perform any service recovery as necessary. Financial * Works in conjunction with the Manager to make recommendations on yearly budgets and financial matters related to department goals and objectives. Successful Growth * Works in conjunction with Manager regarding PAN strategic planning and process improvement across service groupings and continuum of care. * Recommends and support implementation of any new programs or services for department. Performs other duties as assigned Complies with all Wellstar Health System policies, standards of work, and code of conduct. Required Minimum Education:

Required Minimum License(s) and Certification(s): All certifications are required upon hire unless otherwise stated.
    Additional License(s) and Certification(s): Required Minimum Experience: Minimum 3 years Healthcare setting, preferably in a post-acute setting (Home Health, Home Medical Equipment, LTAC, Hospice, IRU, Nursing Home, etc.) Required Required Minimum Skills: Must possess a thorough knowledge of clinical, financial, and qualifying criteria for the post-acute services area. High Must be professional, independent and self-directed. High Strong organizational and communication skills.
