The Cleveland Clinic Florida Care Management team is made up of health care professionals dedicated to helping patients stay on track with their health goals. As an Assistant Nurse Manager in Care Management, you will assist with day-to-day operations, utilize clinical expertise and understanding of care management and Medicare regulations, contribute to the goals of cost containment and quality care and provide safe, appropriate transitions of care.
The hours of this full-time position are 8am-4:30pm plus call requirements to include weekends.
Required qualifications for the ideal candidate include:
· BSN from an accredited school of professional nursing.
· Current state FL licensure as Registered Nurse (RN).
· Professional certification as a Case Manager (CCM)
· 3 years or more of acute care nursing experience
· 2 years ore more of care management or transitional care planning experience.
· Previous supervisory experience.
