JOB DESCRIPTION:

Job Summary

Provides patient care through the provision of direct operational and/or clinical patient services. Pharmacist will collaborate within the various hospital departments to enhance pharmacy operations and clinical programs. Pharmacist will participate in departmental and/or institutional committees, actively participate in the provision of education, and promotion of the profession. This position will require weekend work and holidays (if applicable). The amount of time allocated to operations and clinical services will depend upon hospital requirements, staffing, and overall pharmacy workload. This position also acts as a mentor to facilitate learning among pharmacy/medical students.

The hospital pharmacist collaborates with the multidisciplinary team to improve care, achieve optimal outcomes, and provide continuity of care to patients through the provision of pharmacy patient care services to include therapy care plan development, medication management, education, monitoring and follow up. The pharmacist provides evidence-based and cost-effective comprehensive medication management for patients with chronic conditions.
 

Essential Functions

  • Pharmacist is expected to be knowledgeable and compliant with the hospital's purpose.
  • Provides accurate medication order-entry, drug distribution, and patient care. Confirms the appropriateness of drug therapy as it relates to the care of patients. Seeks to minimize turnaround time for products and services.
  • Actively participates and leads department and pharmacy committees and projects (e.g., performance improvement, formulary management, regulatory, patient safety, financial).
  • Provides concise, applicable, timely responses for drug information requests and other pharmaceutical care services through an on-call service. Provides documentation in the patient care record when appropriate.
  • Actively participates in representing the hospital within a professional organization through the provision of service, presentations, and/or publications.
  • Provides education, evaluation, and training for pharmacy technicians and students as well as medical students/residents and new providers.
  • Provides leadership and pharmaceutical excellence through direct patient care activities as a member of a multidisciplinary medical team.
  • Assists the hospital with its outpatient pharmacy initiatives
  • Performs other duties as assigned.

Supervisory/Management Responsibilities

  • This position will manage a pharmacy technician(s) and will report to a supervisor, manager, director or executive.
  • Will act as Consultant Pharmacist of Record (COR) with the State of FL

Knowledge, Skills and Abilities

  • Knowledge of Automated Dispensing units and the use within a hospital setting
  • Knowledge of medical and pharmacy terminology; possess good mathematical, computer, and aseptic technique skills
  • Knowledge of State BOP/Dept of Health/DEA rules/JCAHO regulations
  • Ability to handle urgent, stressful working conditions

Work Shift

M-F 8-4 (30 min lunch)

Weekends (various)

$108,920.00--$163,400.00
