DEPARTMENT SUMMARY: St. Charles Outpatient Behavioral Health encompasses practices in three Central Oregon communities.  We provide non-urgent outpatient assessment and therapeutic services for all ages.  Issues and concerns that we help with include but are not limited to the following: Depression, Anxiety, Grief, Aging, Marital or family conflicts, Substance Abuse, Stress, Chronic Health/Medical conditions.  We encourage collaboration between medical and behavioral health staff to assure we are providing our community with comprehensive and compassionate behavioral health care.

POSITION OVERVIEW: The Intake Referral Specialist assists in meeting the psychosocial needs of patients and their families . Responsible for helping patients access services to achieve optimum level of functioning. This position does not directly manage any other caregivers.

ESSENTIAL FUNCTIONS AND DUTIES:

Assesses patients' needs and accurately documents information in a timely manner demonstrating compliance with department standards.

Performs patient screening/assessment of social, economic, emotional, and family situations.

Outpatient: Primary responsibility for phone inquiries and client intakes, screening and requests for services for all LCSW, LPC, LMFT, PhD, APP and MD clinic staff. Assesses for appt type, schedules with appropriate clinician and/or psychiatrist and collects information necessary for insurance verification and preauthorization. Coordinates with clinicians and preauthorization staff to insure timely start of services for new clients. 

Functions as a member of the Interdisciplinary Care Team to assure care is accomplished effectively and efficiently in a cost effective manner. 

Establishes and maintains relationship interdepartmentally as well as within assigned department to ensure customer and caregiver satisfaction with referrals/admissions/appointments.

Maintains a thorough knowledge of community resources. 

Processes orders/referrals from IBH providers from SCMG practices.

Provides immediate response to intakes with contracted OPBH specialty medical groups.

Stays informed regarding SCHS Disaster Plan and act as first responder to Disasters for OPBH.

Works with Sage View staff to coordinate aftercare services for Sage View clients.

Coordinates with Emergency Department Social Workers and the Mobile Crises Assessment Team (MCAT) regarding patient mental health needs.

Oversees OPBH claims denial management and coordinate efforts with the CBO.

Provides consultation to clinic providers regarding clinical, ethical, privacy practices and CPT and DSM coding changes and protocols.

Oversees daily operation of the Caregiver Assistance (CAP) and EAP Contracted Provider Programs.

Gathers data and accurately documents information in a timely manner demonstrating compliance with department standards.

Maintains competence with use of Epic and other interfacing EHRs.

EDUCATION:  

Required: Bachelor’s Degree in Social Work, Behavioral Science, or other related field

Preferred: Master’s Degree in Social Work, Behavioral Science, or other related field.     

LICENSURE/CERTIFICATION/REGISTRATION:

Required: Valid Oregon driver's license and ability to meet St. Charles Health System driving requirements.

Preferred: Registered Bachelor’s Social Worker preferred (RBSW). Clinical Licensure with State of Oregon. Member of the National Association of Social Worker’s or other national professional association membership.

           

EXPERIENCE:

Required: Three (3) years Mental Health related experience.

Preferred: One (1) year of experience in a hospital setting. Previous intake and referral experience.

                       

PERSONAL PROTECTIVE EQUIPMENT:

Must be able to wear appropriate Personal Protective Equipment (PPE) required to perform the job safely.

PHYSICAL REQUIREMENTS: 

Continually (75% or more): Use of clear and audible speaking voice and the ability to hear normal speech level.  

Frequently (50%): Sitting, keyboard operation. 

Occasionally (25%): Standing, walking, bending, stooping/kneeling/crouching, climbing stairs, reaching overhead, lifting/carrying/pushing or pulling 1-10 pounds, grasping/squeezing, operation of a motor vehicle, ability to hear whispered speech level. 

Never (0%):  Climbing ladder/step-stool, lifting/carrying/pushing or pulling 11-50 pounds. 

Exposure to Elemental Factors 

Occasionally (25%):  Chemical solution for Laboratory Caregivers. 

Rarely (10%): Wet/slippery area. 

Never (0%):  Heat, cold, noise, dust, vibration, chemical solution, uneven surface.
