Job Description Summary:
OhioHealth is seeking a clinical ambulatory pharmacist to join our team. This pharmacist will work primarily in the hepatology clinic. They will provide evidence-based therapy management and care for patients with advanced liver disease, fatty liver, liver tumors, and metabolic conditions. They will work alongside an interdisciplinary care team in the comprehensive liver program to provide expert care to our patients. The pharmacist is responsible for patient-centered education throughout the course of their therapy, providing appropriate patient instruction and dosing guidance based upon tests and laboratory results, reviewing individualized patient treatment plans, and ensuring patient adherence to medication therapy. This position will support ambulatory needs. Pharmacists will be expected to serve as preceptors for pharmacy students and pharmacy residents in their area of training and expertise.Minimum Qualifications:
Bachelor's Degree (Required)OPL - Ohio Pharmacists License - Ohio Board of PharmacyOhio Board of PharmacyOhio Board of PharmacyBS or Doctor of Pharmacy degree from an accredited institution and a current Ohio pharmacist license. Completion of a Pharmacy Practice residency or equivalent hospital/ambulatory experience.
SPECIALIZED KNOWLEDGE
Pharmacokinetic drug dosing experience; experience with collaborative practice agreements
