A Brief Overview:
The purpose of the Physician FPA is to provide medical direction for the Hospice patient care program according to Hospice policy and procedure. This position reports to the Vice President, Home Services. This position is 55% Health Information Technology, 25% Accountable Care Organization, 20% Medical Group practicing physician.

What you will do:
  • Follows hospital defined policies.
  • Makes recommendations for policy changes pertaining to HIT as business needs change, and applications are enhanced.
  • Clinical Lead for Total Population Health - tools, functionality, and workflows
  • Clinical Lead as assigned for analytics projects including care gap and quality reporting, and risk stratification for care management programs.
  • Clinical consultant as needed for Epic EMR workflows, providing clinical support for workflow design to ensure it meets operational requirements.
  • Clinical consultant as needed for Digital Health regarding tools, functionality, and workflows to promote patient engagement.
  • Participate in the primary care focused ACO Leakage Workgroup; Coordinate ACO leakage activities.
  • Perform the role of the primary physician link between Population Health and VBC initiatives.
  • Develop and drive VBC strategies focused on: Network loyalty/catchment, Care Model Optimization, Care Standardization, and Utilization Management
  • Work in conjunction with CQTO, IPA Medical Director and MD Leadership to develop, implement and manage Physician VBC reporting, Tiering Methodology, and Academic Detailing
  • Participate in the development of High Performing Network Criteria, Timing/Plan and Management
  • As necessary, in partnership with Quality, determine quality measures based on external benchmarks and internal efforts.
  • All duties and responsibilities assigned under the NorthShore Medical Group
  • Maintains department records, reports, and files as required.
  • Attend departmental meetings as required.
  • Enhances professional development through participation in educational programs, current literature reviews, in-services, meetings, and workshops.
  • Promotes the goals, expectations, and policies of the department and organization and represents the department in appropriate clinical and administrative meetings. Demonstrating this ability and achieving a win / win solution while ensuring HIT goals and strategies is a requirement.
  • Carry a pager at all times unless alternative coverage is arranged.
  • Adheres to NorthShore and HIT policies and procedures.
  • Takes on specific tasks as assigned by senior management.

What you will need:
  • Doctorate Medicine Required
  • 5+ Years of experience with Practicing physician in the areas of: EMR optimization, clinical informatics and Total Population Health
  • Strong leadership characteristics and demonstrates self-confidence and is always able to articulate issues.
  • Ability to plan, direct and coordinate all activities of assigned initiatives with minimal supervision.
  • Demonstrated ability in representing the HIT department, its strategies and policies in appropriate administrative and clinical meetings.
  • Demonstrate a high degree of motivation.
  • Is committed to excellence, moderated where appropriate with business sense and practicality.
  • Has a client service mentality and orientation and a bias for action.
  • Possesses managerial skills that empower and develop others; is sensitive to others and fosters teamwork.

  • Demonstrated excellent written and verbal communication skills and the expertise to translate clinical and technical terminology into terms understandable by management, department heads and performance level personnel.
  • An understanding of the healthcare applications suite and their place in the acute care, physician, ambulatory and ancillary settings.
  • Ability to solve highly complex organizational, operational and people problems.
  • Excellent troubleshooting, debugging and problem-solving skills
