Keystone Human Services is currently seeking Intensive Case Managers to join our team in providing individualized, person-directed support in the area of mental health. Our work focuses on inclusion and building communities where everyone has the opportunity to thrive and fully participate in everything society has to offer.

As an Intensive Case Manager, you will assist adults with mental illness gain access to needed resources such as medical, social, educational, and other services with outcomes focused on independent living, vocational and educational participation, adequate social supports, and reduce hospitalization.

Job Details:

• Full time, non-exempt position
• $22.00 per hour

Minimum Educational Qualifications: (one of the following)

• High school diploma with twelve (12) credits in sociology, social welfare, psychology, gerontology, or other social sciences and two (2) years of experience in human services with one (1) year of direct client contact
• Bachelor’s degree in social work, sociology, psychology, gerontology, anthropology, criminal justice, theology, counseling, education, or nursing
• Registered Nurse licensure

Additional Minimum Requirements:

• Effective communication and proficient computer skills
• Valid driver’s license with daily access to a privately maintained and insured vehicle
• Successful completion of the pre-employment process, including clearances and verifications
