Position Highlights:

In collaboration with the attending physician:

  • Formulates the appropriate differential diagnosis based on history, physical examination and clinical findings
  • Identifies needs of the individual, family or community as a result of the evaluation of the collected data
  • Obtains a relevant health and medical history                                                                                                                    
  • Performs a physical examination based on age and history                                                                                                                           
  • Conducts preventative screening procedures based on age and history                                                                                                                   
  • Identifies medical and health risks and needs                                                                                                                     
  • Updates and records all changes in health status                                                                                                                   
  • Formulates the appropriate differential diagnosis based on history, physical examination and clinical findings                                                                                                                            
  • Identifies needs of the individual, family or community as a result of the evaluation of the collected data                
  • The nurse practitioner facilitates client participation medical and health care by providing information needed to make decisions and choices about the:
  • The nurse practitioner provides educational opportunities for the individual and/or family related to health status, utilizing additional resources when indicated.

What you will need:

  • Completion of an accredited Nurse Practitioner or Physician Assistant Program       
  • Certified Nurse Practitioner or Physician Assistant
  • IL Nurse Practitioner or Physician Assistant license or pending
  • Valid DEA certificate
