Position Highlights:

  • Location:4201 Winfield Rd, Warrenville, IL
  • Full Time 40 hours per week
  • Hours: Monday-Friday 8-430

What you will do:

  • Demonstrates the knowledge and skills necessary to provide care appropriate to the age of patients served. This includes knowledge of the physical and psychological needs of patients served and the ability to respond appropriately to those needs.
  • Performs care management duties toward targeted patient care, and appropriate resource utilization goals and targets. Guide the team to provide safe and effective care progression and reduce risk of readmission for patient population.
  • Identifies needs for patients and their families, coordinates with members of the health care team to develop and modify care plans, and provide creative guidance in initiating care plan and in overcoming any self-management challenges.
  • Works with Community Resources to ensure resources available to the patient and their disease specific care options.
  • In conjunction with other members of the health care team, coordinates care for patients with complex needs and facilitates post discharge planning. Works closely with Utilization Review Coordinators to provide necessary information to payers to justify admission and continued stay.
  • Enhances patient satisfaction facilitating interaction and communication among members of the health care team and problem solving with staff and physicians. Queries patients and families to identify unmet needs and opportunities for improvement.
  • Serves as a clinical expert/resource to staff with respect to resource utilization and patient care. Problem solves with staff to aid in the development of critical thinking skills.
  • Participates in the design and implementation of pathways and other clinical improvement activities
  • Mentors staff and provides informal staff education and role modeling.

What you will need:

Education, Experience and/or Skills Required:

  • Bachelor’s Degree in Nursing or Bachelor’s Degree in health care related field with care management experience.
  • Two years of experience in an outpatient ambulatory care setting.
  • Current State of Illinois Registered Nurse licensure
  • Current CPR certification issued by the American Heart Association or American Red Cross

Education, Experience and/or Skills Preferred:

  • Certification in Case Management
  • One year previous case management/care management and/or clinical specialist experience
  • Epic User experience preferred
