About the Position:

YAI has an opening for a Full-Time Individual Service Coordinator (ISC) to work within our Residential programs in Westchester County, NY. The Individual Service Coordinator (ISC) at YAI provides case coordination and administrative support and serves as an advocate for clients between program, families, residences, and clinical/care managers.

Location: Westchester County, NY

Schedule: Part-Time

Key responsibilities:

- Manage a caseload of around 75+ individuals

- Input services and meeting minutes into E-Vero

- Taking notes in meetings

- Ability to communicate with all families/residences despite challenges

- Understanding of clinical needs of clients

- Liaison between families/residences/clinicians/care managers 

- Support BIS and other staff/clinicians with client care

- Maintain records in accordance with OPWDD regulations

- Maintain compliance of program in accordance with OPWDD regulations

- Assist with audits

- Assess and implement staff action plans

- Entering and managing databases

- Provide coverage based on programs needs

Education and Experience:

- BA preferred

- Experience working with people with intellectual and developmental disabilities and advocating for their needs/desires required

Additional Knowledge, Skills, and Abilities:

- Exceptional interpersonal, problem solving, organizational, presentation and communication skills

- Willing to take initiative

- Highly flexible personality type

- Able to deal with challenging families/residences

- Understanding of person-centered philosophy

- Knowledge of E-Vero

- Ability to work in Team Environment

Compensation:

- $18.31/hour- $18.31/hour
