Job Description:
Schedule: 7 Days On, 7 Days Off (Work One Week, Take the Next Off)
Monday-Thursday: 5:00 PM - 8:00 AM
Friday: 5:00 PM - Monday 8:00 AM
Coverage Area: Lafayette, IN

Patient Assessment: Evaluate home health care patients to identify their physical, psychosocial, and environmental needs.
Care Planning: Make initial evaluations and re-evaluate nursing needs during each visit.
Communication: Report significant findings, problems, and changes in health conditions to the clinical supervisor, physician, and other care personnel.
Patient Counseling: Provide safety plans and utilize family and community resources for patient care.
Care Implementation: Ensure quality and continuity of care for an assigned case load to achieve patient outcomes.
Action Initiation: Identify and act on deviations from normal nursing and patient condition statuses.
Plan Verification: Verify the Plan of Care before each visit and deliver care according to physician’s orders, assessment data, and established guidelines.
Graduate of an accredited Certificate, Diploma, Associates or Baccalaureate School of Nursing.
1+ year of experience as a nurse in a clinical care setting.
Home Health experience is preferred, but not required.
Current State license as a Registered Nurse.
Ability to travel approximately 50%
Excellent verbal and written communication skills.
Preferred to live in the service area
