Position Summary
The Pharmacist works collaboratively with all members of the care team providing world class service to both the patients and staff of Martin’s Point. The Staff Pharmacist ensures the proper compounding, dispensing, review and verification of prescription medications within regulatory guidelines, company policies and procedures; provides appropriate drug information to patients and clinical staff; and actively supports organizational/departmental initiatives in an accurate and professional manner..
Job Description
Key Outcomes:
- Meets patient satisfaction and medication error rate metrics while accurately performing the prescription dispensing process in accordance with federal, state, JCAHO, MPHC and Department of Defense rules and regulations.
- Ensures proper drug usage counseling is provided to patients, following OBRA regulations.
- Verifies that prescriptions prepared by the technician staff are filled accurately.
- Consults and educates patients, providers and appropriate staff regarding current drug information and pharmacy initiatives.
- Outreaches to medical providers to clarify prescribed medications, dosages, refills, interactions and allergies to suggest alternative medications and answer medical provider questions.
- Complies with Board of Pharmacy regulations, Federal and State Law, JCAHO standards and Department/organizational policies and procedures.
- Reports and documents clinical pharmacy activities, including but not limited to recommendations and outcomes.
- Ensures the accurate processing of insurance claims to prevent payment rejections. Follows-up with insurance companies and medical providers and participates in 3rd party audit.
- Assists the Pharmacy Manager with identifying and managing potential problems in such areas as formulary compliance, generic substitution, prescribing patterns, inventory management, customer experience, etc.
- Performs pharmacy interventions in alignment with Health Plan and/or Delivery System strategic goals ex. Medical Expense Management)
- Performs special projects, as assigned.
- Participates in huddles and regular team meetings to improve workflows and contribute to improving patient population outcomes.
- Assists other MPHC pharmacy departments as required, including but not limited to adherence outreach and Medication Therapy Management.
Education/Experience:
- Bachelor’s Degree in Pharmacy or Pharm. D
- 2 years experience as a pharmacist in an outpatient clinic, mail order or retail setting.
- HMO/managed care pharmacy experience preferred.
License/Certification:
- Registered pharmacist in the state in which employee practices (i.e. Maine, New Hampshire or both)
- Current and active immunizing pharmacist certification (Health Center locations only)
