What you will need:

The Advance Practice Provider, APRN or PA, functions as part of a multidisciplinary health team in accordance with privileges approved by the credentialing committee to provide high-quality, cost-effective advanced care to patients. The APP is responsible for the health outcome of an assigned patient population and consults, confers, and collaborates as needed to ensure the best patient outcomes. Provides professional patient care in the Outpatient Infusion Center on an off-campus site, including performing problem focused histories and physical examinations, interprets laboratory tests, and diagnostic tests. Interprets and correlates subjective and objective data. Has ability to formulate a differential diagnosis and establishes a working diagnosis for each patient.

The Advance Practice Provider, reflects the mission, vision, and values of Swedish Hospital and Endeavor, adheres to the organization’s Code of Ethics and Corporate Compliance Program and complies with all relevant policies, procedures, guidelines, and other regulatory and accreditation standards.

Essential Functions and Responsibilities':

-Reviews treatment plan for medical necessity.

-Reviews patient’s assessment and history.

-Reviews appropriate diagnostic imaging studies, test and labs. Interpret findings.

-Orders appropriate diagnostic imaging studies, test and labs as needed in collaboration with referring provider.

-Address patient questions and concerns.

-Assess patient as needed.

-Communicates abnormal lab and imaging results as appropriate and in a timely manner.

-Collaborate and delegate to care team as appropriate

-Oversees medication administration and is available to manage suspected drug reactions.

-Provides medication orders for emergent administration and attends the patient.

-Determines need for emergent medical transport

-Ensuring that activation of EMS was done appropriately and if needed, relaying relevant  information to the receiving team at the hospital (whether it be the ED or a direct admit).

-Reports reactions to the referring physician

-Documents all care, assessments and interventions in the EHR system in a timely manner.

- Returns patient phone messages if needed, in accordance with practice standards, and documents actions taken in the patient record.
