Job Summary

The Clinical Pharmacy Coordinator will organize, implement, direct and coordinate all pharmacy-related activities of the designated program.

Job Description

Essential Functions:

  • Acts as a clinical pharmacy resource in area of specialty.
  • Teaching and education: Participates in ongoing teaching and education of staff, students and post-graduate learners.
  • Quality Assurance/Performance Improvement:  Identify, participate or lead quality improvement projects. 
  • Clinical Pharmacy Practice: Providing oversight of clinical pharmacy practice; Collects and applies clinical data to meet department needs; Ensure medication use is aligned with evidence-based best practices; Under general direction/guidance from leadership, leads development, implementation, and maintenance of medication services in a specialty area (e.g. medication safety, drug shortages, formulary changes, team leader in a specific practice area); Participate in collaborative projects with multidisciplinary teams to improve patient care, quality improvement or resolve clinical problems.
  • Policies and Procedures:  Participate in policy and procedure development and review to assure conformance with applicable standards of practice.
  • Regulatory:  Participate in regulatory requirements and reviews (e.g. Investigational drug trials, Joint Commission, Department of Health, Board of Pharmacy, ASHP Residency, Accreditation Council for Graduate Medical Education (ACGME)).
  • All other duties as assigned.

Oncology Program:

  • Performs duties of Clinical Pharmacist.
  • Manage investigational drug research pharmacy activities.

Family Medicine Residency Program:

  • Family Medicine Residency Program including teaching and evaluation of resident physicians and other post-graduate learners, proper medication selection and monitoring of drug therapy; research and scholarly activity and compliance with all ACGME and other regulatory standards.
  • Participates effectively on multidisciplinary patient care rounds.
  • Serves as member of FMR Clinical Competency Committee providing formal feedback regarding Residency Physicians’ performance, per ACGME regulations.
  • Serves as member of FMR Faculty Committee which provides operational oversight of the educational program and clinical activities.

Specialty Pharmacy:

  • Collaboratively develop, implement, and oversee the Monument Health Home+ Specialty Pharmacy (MSP) Embedded Clinic Pharmacist Program. Provide direct patient care to establish medication regimens and collaborative disease state management.
  • Collaboratively develop, implement, and optimize the MSP Therapy Management Program. Contribute to Quality Management Committee.
  • Perform staffing duties of specialty clinical pharmacist.

Additional Requirements

Required:
Education - Doctorate degree in Pharmacy
Certification - Pharmacist - South Dakota Board of Pharmacy; Board Certified Oncology Specialist (BCOP) Certification - Board of Pharmacy Specialties (BPS)

Preferred:

Experience - Oncology (PGY-2 Oncology Residency), Family Medicine (HIV certification)

Physical Requirements:
Light work - Exerting up to 20 pounds of force occasionally, and/or up to 10 pounds of force frequently, and/or a negligible amount of force constantly to move objects. Possible exposure to blood, bodily fluids, or tissues.
